Forensic toolmark examiners have been comparing features observed in toolmarks to help determine their source for over a century. However, the past decade, holistic process of and presenting findings court faced intense scrutiny. This paper provides summary voiced criticisms, primarily concerning scientific reliability validity comparison methods employed by conclusions they testify to. Background: Firearm wounds tend to have a precise pattern. Despite this, real-world case presentations can present uncertain elements, sometimes deviating from what is considered standard, and uncommon features that are difficult for forensic pathologists ballistic experts explain. Methods: A retrospective analysis of autopsy reports the Institute Legal Medicine, University Catania, covering 2019–2023, included 348 judicial inspections 378 autopsies performed as part institute’s overall activities. Among these, seventeen cases firearm deaths were identified, with three atypical selected detailed analysis. An interdisciplinary approach involving pathology, radiology, ballistics was used.
