Background Despite remarkable benefits have been provided by immune checkpoint inhibitors in gastric cancer (GC), predictions of treatment response and prognosis remain unsatisfactory, making identifying biomarkers desirable. The aim this study was to develop validate a CT imaging biomarker predict the immunotherapy patients with GC investigate associated infiltration patterns. Methods This retrospective included 294 who received anti-PD-1/PD-L1 from three independent medical centers between January 2017 April 2022. A radiomics score (RS) developed intratumoral peritumoral features on pretreatment images immunotherapy-related progression-free survival (irPFS). performance RS evaluated area under time-dependent receiver operating characteristic curve (AUC). Bulk RNA sequencing tumors 42 Cancer Genome Atlas RS-associated Results Overall, 89 (median age, 57 years (IQR 48–66 years); 171 males) had an objective immunotherapy. 13 that yielded AUCs 12-month irPFS 0.787, 0.810 0.785 training, internal validation, external validation 1 cohorts, respectively, AUC 24-month 0.805 2 cohort. Patients low longer each cohort (p<0.05). analyses showed is prognostic factor integrated clinical characteristics improved predicting irPFS, 0.687–0.778 training cohorts. M1 macrophage infiltration.
