The impact-intensive and rapidly growing pharmaceutical industry must ensure its sustainability. This study reveals that environmental sustainability assessments have been conducted for only around 0.2% of pharmaceuticals, impacts significant variations among the assessed products, different impact categories not consistently studied. Highly varied require assessing more products to understand industry's status. Reporting all will be crucial, especially when comparing production technologies. Biological (semi)synthetic pharmaceuticals could reduce their costs, though high biologically produced monoclonal antibodies should also optimized. Considering potential biopharmaceuticals from economic, environmental, social perspectives, collaboratively guiding immense market growth would lead transition.
