Introduction Prolonged hyperglycemia caused by type 2 diabetes mellitus (T2DM) can lead to liver injury and disrupt the community of gut microbiota that pose significant risks human health. As a food rich in variety active ingredients, Zingiber striolatum ( Z. ) exhibits hypoglycemic hypolipidemic effects. However, regulatory influence ethanol extract (ZSE) on T2DM mice or its potential relationship with pathology remains unexplored. Methods After one-week acclimation period, 12 were randomly selected as normal group. The remaining 48 employed model, then assigned four groups: model group, low-dose ZSE group (ZSE-L, 100 mg/kg/day), high-dose (ZSE-H, 300 positive control treated metformin hydrochloride (MET, mg/kg/day). Results 4-week intervention, results revealed significantly ameliorated fasting blood glucose (FBG), area under curve oral tolerance test (AUC OGTT) glycated serum protein (GSP) mice. Moreover, (ZSE-H) treatment increased relative abundance beneficial bacteria such Faecalibaculum , while reducing harmful Bilophila thereby alleviating insulin resistance. Additionally, ZSE-H demonstrated superior efficacy over (ZSE-L) improving FBG, AUC OGTT, other parameters. Predictive analysis correlation between parameters identified Dubosiella Bacillus Mailhella microbial biomarkers for further investigation into pathogenesis T2DM. Conclusion plays pivotal role mitigating through modulation intestinal communities.
