Purpose: Chronic venous leg ulcers (CVLUs) comprise the majority of lower-extremity wounds, yet their pathophysiology is not fully understood. While research has shown that microRNAs are an important component wound inflammation, few have explored role (miRNAs) in healing CVLUs. This scoping review examines miRNAs CVLUs and association with healing. Methods: In December 2023, we searched MEDLINE/PubMed, Embase, Scopus, CINAHL for studies published 2013–2023 examining CVLU Results: Six met inclusion criteria. MicroRNAs were extracted from various specimens including serum, skin biopsy samples, adipose tissue-derived mesenchymal cells individuals Overexpression miR-221, miR-222, miR-92a, miR-301a-3p hindered angiogenesis, while overexpression miR-296, miR-126, miR-378, miR-210 facilitated angiogenesis. miR-34a/c, miR-301a-3p, miR-450-5p, miR-424-5p, miR-516-5p, miR-7704 increased local inflammatory responses inhibited keratinocytes proliferation, impairing healing, miR-19a/b miR-20 downregulated keratinocytes' response, promoting Downregulation miR-205, miR-96-5p, miR-218-5p enhanced cellular proliferation promoted miR-17–92 was linked impaired Discussion: play a regulating responses, cell migration chronic-wound However, limited lack standardized approach to measurement quantification. Further warranted elucidate mechanisms underlying microRNA involvement better understand future development targeted therapies.

Non-healing wounds are long-term complications of diabetes mellitus (DM) that increase mortality risk and amputation-related disability decrease the quality life. Nitric oxide (NO·)-based treatments (i.e., use both systemic topical NO· donors, precursors, inducers) have received more attention as complementary approaches in DM wounds. Here, we aimed to highlight potential benefits NO·-based on through a literature review experimental clinical evidence. Various been used. In rodents, therapy facilitates wound healing, manifested an increased healing rate decreased half-closure time. The effect is attributed increasing local blood flow, angiogenesis induction, collagen synthesis deposition, re-epithelization, anti-inflammatory anti-oxidative properties, potent broad-spectrum antibacterial effects. existing lacks human evidence safety efficacy for Translating favors into practice needs conducting trials with well-predefined sizes, i.e., reduction area, hospital length stay.

The dysfunction of wound-healing processes can result in chronic non-healing wounds and pathological scar formation. Current treatment options often fall short, necessitating innovative approaches. Exosomes, extracellular vesicles secreted by various cells, have emerged as promising therapeutic agents serving an intercellular communication system. By engineering exosomes, their cargo surface properties be tailored to enhance efficacy specificity. Engineered exosomes (eExo) are emerging a favorable tool for treating scars. In this review, we delve into the underlying mechanisms scars, outline current state strategies, explore clinical potential eExo based on preclinical studies. addition, address challenges future research directions, including standardization, safety assessments, immune responses. conclusion, hold great promise novel approach Further trials warranted translate findings effective treatments.
