Triglyceride-glucose (TyG) index has recently been established as an indicator of insulin resistance and predictive value for cardiovascular (CV) disease. However, the clinical significance TyG in patients undergoing hemodialysis remains unknown. We prospectively enrolled 759 maintenance hemodialysis. The Cardiometabolic Index (CMI), a novel metabolic marker, has been associated with various diseases in previous studies. However, its relationship stroke risk remains underexplored. This investigation sought to elucidate predictive capabilities of CMI outcomes explore underlying mechanistic pathways establish comprehensive risk prediction framework. The study encompassed 7,822 individuals from national health retirement longitudinal cohort, with participants stratified by quartiles. Following baseline characteristic comparisons CVD incidence rate calculations, we implemented multiple Cox regression models assess CMI's capabilities. For nomogram construction, utilized an ensemble machine learning framework, combining Boruta algorithm-based feature selection Random Forest (RF) XGBoost analyses determine key parameters.
