This systematic review with trial sequential analysis (TSA) aims to evaluate the efficacy and safety of Shu Mian Capsule (SMC), a commercial Chinese polyherbal preparation, for managing depression or without comorbid insomnia. Controlled clinical trials assessing SMC against waitlist control, placebo active controls, as an adjunct treatment were searched across seven databases. Risk bias evidence quality assessed using Cochrane criteria GRADE framework, respectively. Fourteen studies analyzed, involving 1207 participants. Trials comparing standard antidepressive treatments limited. In depressed patients insomnia, combining antidepressants reduced incidence antidepressants-induced sleep disorders (from 12.2% 3.8%) but did not significantly lower Hamilton Rating Scale Depression (HAM-D) scores compared alone [SMD = -0.09, 95% CI (-0.32, 0.14), p 0.45]. combination psychotropic drugs HAM-D -1.29, (-1.96, -0.62), < 0.01] Pittsburgh Sleep Quality Index -1.53, (-1.95, -1.11), 0.01], exhibited various drug-related adverse effects alone. TSA validated sample size adequacy; nevertheless, methodological supporting varied from very low due substantial risk. Additionally, 92.9% lacked follow-ups. The effectiveness alternative conventional is unclear. For adding care demonstrates augmented improved safety, though methodologically Further rigorous are warranted confirm SMC's short-term explore its medium- long-term either complementary therapy. Current precludes recommendations administration in depression.

Ischemic stroke (IS) is one of the most fatal diseases. Neuroimmunity, inflammation, and oxidative stress play important roles in various complex mechanisms IS. In particular, early proinflammatory response resulting from overactivation resident microglia infiltration circulating monocytes macrophages brain after cerebral ischemia leads to secondary injury. Microglia are innate immune cells that constantly monitor microenvironment under normal conditions. Once occurs, activated produce dual effects neurotoxicity neuroprotection, balance two determines fate damaged neurons. The activation defined as classical (M1 type) or alternative (M2 type). M1 type secrete pro-inflammatory cytokines neurotoxic mediators exacerbate neuronal damage, while M2 promote a repairing anti-inflammatory response. Fine regulation M1/M2 microglial minimize damage maximize protection has therapeutic value. This review focuses on interaction between other involved IS phenotypic characteristics, mechanism natural plant components regulating IS, providing novel candidate drugs for drug development.

Adult hippocampal neurogenesis generates functional neurons from neural progenitor cells in the dentate gyrus (DG) to complement and repair circuits, thus benefiting treatment of depression. Increasing evidence has shown that aberrant microglial activity can disrupt appropriate formation development properties neurogenesis, which will play a crucial role occurrence However, mechanisms crosstalk between microglia adult depression are not yet fully understood. Therefore, this review, we first introduce recent discoveries regarding roles etiology Then, systematically discuss possible how regulate according studies, involve toll-like receptors, polarization, fractalkine-C-X3-C motif chemokine receptor 1, hypothalamic-pituitary-adrenal axis, cytokines, brain-derived neurotrophic factor, microbiota-gut-brain etc. In addition, summarize promising drugs could improve by regulating microglia. These findings help us understand complicated pathological shed light on new strategies for disease.

Depression has become one of the most commonly prevalent neuropsychiatric disorders, and main characteristics depression are sleep disorders melatonin secretion caused by circadian rhythm disorders. Abnormal endogenous alterations can contribute to occurrence development depression. However, molecular mechanisms underlying this abnormality remain ambiguous. The present review summarizes antidepressant effects melatonin, which is related its functions in regulation hypothalamic-pituitary-adrenal axis, inhibition neuroinflammation, oxidative stress, alleviation autophagy, upregulation neurotrophic, promotion neuroplasticity levels neurotransmitters, etc. Also, receptor agonists, such as agomelatine, ramelteon, piromelatine, tasimelteon, GW117, have received considerable critical attention highly implicated treating comorbid This focuses on various agonists pathophysiology treatment depression, aiming provide further insight into pathogenesis explore potential targets for novel agent development.
