Abstract Delirium is a common and serious neurobehavioral syndrome, associated with prolonged hospital stays, increased morbidity mortality. As it remains unclear whether suvorexant or without ramelteon prevents delirium in elderly hospitalized patients, we conducted systematic review meta‐analysis to evaluate, searching the PubMed, Cochrane Library, Web of Science, EMBASE, EBSCOhost databases for all randomized controlled trials (RCTs), case–control studies, cohort studies that investigated effects on adult patients. The primary outcome was incidence delirium. Two trials, 7 2 involving 2594 patients were included this meta‐analysis. results showed both alone (odds ratio (OR) = 0.30, 95% confidence interval (CI): 0.14–0.65, P 0.002) (OR 0.39, CI 0.23–0.65, 0.0003) reduced Six involved use benzodiazepines; subgroup analysis performed separately groups indicated when benzodiazepine administered, effective at reducing 0.53, 0.37–0.74, 0.0002), but no significant difference observed 0.40, 0.11–1.53, 0.18). current literature thus supports effectiveness prevention, although failed significantly reduce administered. present study limited by heterogeneity among caution should be exercised interpreting results. Sleep disturbance among hospitalized patients has many factors especially at critical care units. Daily breathing exercises as pursed lip (PLB) and diaphragmatic improves sleep quality reduces the occurrence of delirium. Aim study Evaluate effectiveness using exercise on patients. Sample A purposive sample including 152 were divided into four equal groups two from Medical ward (study control) 38 each group form Intensive Care Unit (MICU) Minia university hospital. Introduction: Delirium, being a common disorder among critically ill patients, has reported incidence ranging 45-83% in the intensive care unit (ICU) population.The prophylactic use of melatonin and melatonergics been shown to have positive effect reducing delirium many clinical trials.Our study was thus proposed find out role on severity ICU delirium, hospital length stay (LOS), requirement, duration mechanical ventilation, mortality.Methodology: A systematic search various databases performed trials which compare with placebo or standard therapy for prevention results conveyed as mean differences (MDs) risk ratios.The statistical software, Review Manager (RevMan, version 5.4), used data synthesis.Results: Twelve studies were included meta-analysis.Prophylactic administration ramelteon not associated statistically significant reduction (odds ratio [OR] 0.63; confidence interval [CI]: 0.60, 1.32; p = 0.22), (MD: 0.22; 95% CI: From -1.36 1.81; 0.78), LOS 0.05; -0.65 0.75; 0.89), -1.46; -4.50 -1.59; 0.35), need ventilation (OR: 0.74, 0.38-1.44;p 0.37), mortality 0.78; 0.56; 1.11; 0.62).However, -0.85; -1.64 -0.06; 0.03) found.Conclusion: Our meta-analysis suggests that when given prophylactically no LOS, mortality.Further are warranted.
