The concept of intensive care units (ICU) has existed for almost 70 years, with outstanding development progress in the last decades. Multidisciplinary critically ill patients become an integral part every modern health system, ensuing improved and reduced mortality. Early recognition severe medical surgical illnesses, advanced prehospital organized immediate trauma centres led to a rise ICU patients. Due underlying disease its need complex mechanical support monitoring treatment, it is often necessary facilitate bed-side diagnostics. Point-of-care real-time reverse-transcription polymerase chain reaction (RT-PCR) facilitates the widespread use of rapid, accurate, and cost-effective near-patient testing that is available to public. Here, we report ultrafast plasmonic nucleic acid amplification quantification for decentralized molecular diagnostics. The RT-PCR system features an thermocycler (PTC), a disposable plastic-on-metal (PoM) cartridge, ultrathin microlens array fluorescence (MAF) microscope. PTC provides photothermal cycling under white-light-emitting diode illumination precise temperature monitoring with integrated resistance detector. PoM thin film cartridge allows rapid heat transfer as well complete light blocking from excitation source, resulting in highly efficient PCR quantification. Besides, MAF microscope exhibits close-up high-contrast microscopic imaging. All systems were fully packaged palm size point-of-care testing. demonstrates diagnosis coronavirus disease-19 RNA virus within 10 min yields 95.6% efficiency, 96.6% classification accuracy preoperational test, 91% total percent agreement clinical diagnostic test. compact can decentralize primary care developing countries.
