ABSTRACT Microscopic colitis is a chronic inflammatory bowel disease characterized by non-bloody diarrhea that can range from mild to severe. It difficult attribute up 10-20% of microscopic colitis. The three determinants factors the diagnosis are characteristic clinical symptoms, normal endoscopic picture colon, and pathognomonic histological picture. This manuscript aimed update considerations recommendations for professionals involved (gastroenterologist, endoscopists pathologist) in MC. In addition, short recommendation about treatment.

Celiac disease (CeD) is a chronic immune-mediated condition triggered by gluten consumption in genetically predisposed individuals. Approximately 1% of the general population affected disorder. Disease presentation heterogeneous and, despite growing awareness among physicians and public, it continues to be underestimated. The most effective strategy for identifying undiagnosed CeD proactive case finding through serologic testing high-risk groups. We reviewed recent evidence on association between more than 20 conditions. In light this review, screening recommended individuals with (1) autoimmune accompanying symptoms suggestive CeD; (2) diseases that may mimic (eg, irritable bowel syndrome [IBS], inflammatory [IBD], microscopic colitis); (3) patients conditions high prevalence: first-degree relatives, idiopathic pancreatitis, unexplained liver enzyme abnormalities, hepatitis, primary biliary cholangitis, hyposplenism or functional asplenia severe bacterial infection, type 1 diabetes mellitus, Hashimoto's thyroiditis Graves' disease, Sjögren's syndrome, dermatitis herpetiformis, recurrent aphthous enamel defects, ataxia, peripheral neuropathy, delayed menarche premature menopause, Down Turner Williams fatigue IgA nephropathy, deficiency. serology should initial step process. However, any aforementioned disorders who are undergoing upper endoscopy, biopsies performed rule out CeD.

The gastrointestinal tract (GI) has a unique oxygenation profile. It should be noted that the state of hypoxia can characteristic both normal and pathological conditions. Hypoxia-inducible factors (HIF) play key role in mediating response to hypoxia, they are tightly regulated by group enzymes called HIF prolyl hydroxylases (PHD). In this review, we discuss involvement inflammation signaling pathways pathogenesis inflammatory bowel disease (IBD) elaborate detail on multiple immune reactions during intestinal inflammation. We emphasize critical influence tissue microenvironment highlight existence overlapping functions responses mediated same molecular mechanisms. An association has been reported between celiac disease (CD) and microscopic colitis (MC). However, large, population-based cohort studies are rare.To systematically examine the CD MC in a nationwide cohort. We conducted matched study Sweden of 45,138 patients with biopsy-verified (diagnosed 1990-2016), 223,149 reference individuals, 51,449 siblings patients. Data on were obtained from all (n = 28) pathology departments Sweden. Adjusted hazard ratios (aHRs) calculated using Cox regression.During follow-up, 452 197 individuals received an diagnosis (86.1 vs. 7.5 per 100,000 person-years). This difference corresponded to aHR 11.6 (95% confidence interval [CI] 9.8-13.8) or eight extra cases 1000 followed up for 10 years. Although risk was highest during first year follow-up (aHR 35.2; 95% CI 20.1-61.6), it remained elevated even after years 8.1; 6.0-10.9). Examining subtypes lymphocytic (LC) collagenous (CC) separately, 12.4 10.0-15.3) LC 10.2 7.7-13.6) CC. also more common before (adjusted odds ratio [aOR] 52.7; 31.4-88.4). Compared siblings, estimates decreased but (CD later MC: HR 6.2; earlier aOR 7.9).Our demonstrated very strong increased future previous The magnitude associations underscores need consider concomitance these diagnoses which gastrointestinal symptoms persist recur despite gluten-free diet conventional treatment. comparatively lower sibling comparisons suggest that shared genetic early environmental factors may contribute MC.

Abstract Background This study was to explore the relationship between chronic constipation, diarrhea, and frailty in older Americans. Methods cross-sectional selected a total of 4241 community-dwelling individuals aged 60 years from 2005–2010 National Health Nutrition Examination Survey. Frailty measured using 49-item index, index > 0.21 defined as frail status. Chronic constipation diarrhea were “usual or most common type stool” by Bristol Stool Form Scale (BSFS) Types 1 2 BSFS 6 7, respectively. Weighted logistic regression analysis used examine gut health Restricted cubic spline (RCS) curves built assess association stool frequency. Results status associated with higher odds an unadjusted model; however, after further adjusting for confounding variables, not statistically significant. We discovered positive correlation adjustment all variables. The showed U-shaped frequency, smallest at frequency 10 stools/week. Conclusion Negative associations observed among adults. Older adults who have bowel movement about times per week are least frail. Future studies warranted confirm causal this association.

We conducted retrospective analysis by gathering case reports series through database search, collected until September 30, 2024. A total forty patients (21 males 19 females) fulfilled inclusion criteria, median age 42 years (range 19, 70). The time onset was 4 months following initial dose 0.25, 53). most common symptoms reported were abdominal pain (60.0%), diarrhea (37.5%), bloody (32.5%), fever (30.0%). Colonoscopy findings predominantly showed ulcers (62.5%) inflammation (27.5%). Biopsy results revealed crypt microabscesses (22.5%), cryptitis (20.0%), inflammatory cellular infiltrates both acute chronic (22.5%). After discontinuation symptom relief treatment systemic steroids targeted therapies (52.5%), mesalamine Comprehensive screenings are essential prior initiating secukinumab therapy.
