Abstract Cannabis use is highly prevalent and associated with adverse beneficial effects. To better understand the full spectrum of health consequences, biomarkers that accurately classify cannabis are needed. DNA methylation (DNAm) an excellent candidate, yet no blood‐based epigenome‐wide association studies (EWAS) in humans exist. We conducted EWAS lifetime (ever vs. never) using DNAm data from a case–cohort study within Sister Study, prospective cohort women at risk developing breast cancer (Discovery N = 1,730 [855 ever users]; Replication 853 [392 users]). identified replicated cg15973234 (CEMIP): combined p 3.3 × 10 −8 . found overlap between published cis‐meQTLs reported use‐associated single nucleotide polymorphism (SNPs; < .05), suggesting observed difference was driven by exposure. also developed multi‐CpG classifier penalized regression top CpGs.
