This study aims to analyse the efficacy of GBE administration in patients with glaucoma and healthy volunteers. systematic review was performed according PRISMA 2020 guidelines. All clinical studies investigating on intraocular pressure (IOP), corrected pattern standard deviation (CPSD) mean visual field testing, heart rate were considered. The weighted difference (MD) effect measure, 95% confidence interval (CI), t-test used for continuous variables. Data from 8 studies, including 428 patients, retrieved. age all 51.1 ± 15.5 years. median follow-up 3.7 (IQR 9.4) months. not associated an improvement IOP (MD -1.5; 95%CI -7.1 9.6; P = 0.5), 0.7; -9.4 8.2; 0.8), CPSD -1.6; -3.8 6.9; or -2.5; -11.5 16.5; 0.4) baseline last follow-up. To compare the efficacy and safety of combined phacoemulsification with ab interno canaloplasty (Phaco-AbiC) versus single-use dual blade goniotomy (Phaco-DBG). In this prospective consecutive case series, intraocular pressure (IOP), number IOP-lowering medications, mean deviation (MD) visual field testing, surgery-related complications were evaluated in 25 eyes following Phaco-AbiC 20 Phaco-DBG through 12 months follow-up. Qualified or complete success was defined as postoperative IOP lower than 21, 18, 16 mmHg without medications. At 12-month follow-up, significantly reduced compared to baseline (14.2 ± 2.7 19.2 4.7 mmHg, p < 0.001) groups (16.7 3.4 19.9 3.9 0.001). higher (p = 0.011). Compared baseline, medications (1.2 1.4 2.4 1.0, (0.8 0.9 2.1 0.9, at For qualified rate (36.0 76.2%, respectively) (17.5 40.6%; 0.037; 0.007, respectively). No severe intra- occurred. Both achieved a significant reduction Phaco-DBG.

Purpose Glaucoma is one of the leading causes irreversible blindness, characterized by progressive visual field loss. Several risk factors are associated with developing disease. However, exact mechanisms or pathological pathways involved still unknown. There an urgent need to find and biomarkers for early detection therapy halt progression cure Extracellular vesicles (EVs), specifically exosomes, have emerged as a crucial player in all aspects glaucoma, including pathogenesis therapeutic application their cell-cell communication properties.
