Intrinsic capacity (IC), a crucial indicator for the United Nations Decade of Healthy Ageing 2021-2030, is defined by WHO as foundation functional ability, representing composite all physical and mental capacities an individual. IC spans five function domains: Locomotor, psychological, cognitive, vitality, sensory (including vision hearing). Accurate assessment vital effective interventions, yet comparative analyses these tools are scarce. Consequently, we evaluated diagnostic accuracy three in individuals aged 80 above-Integrated care older people (ICOPE) Step 1, ICOPE 2, Lopez-Ortiz's scoring system. This cross-sectional analysis included total 475 participants ≥80 years between July 2023 January 2024 11 nursing homes Ningbo, Zhejiang Province, China. Abstract Background Falls in older adults significantly impact overall health and healthcare costs. Intrinsic capacity (IC) reflects functional reserve is an indicator of healthy aging. Prior to this cross-sectional study, the intrinsic capacity (IC) loss rates and protective factors in nursing homes for individuals aged 80 older remained unexplored. Analysing 434 participants, study found 86.9 % of experienced at least one IC domain, with detailed losses locomotion, vision, vitality, hearing, psychological, cognitive capacities 83.2 %, 52.8 50.9 46.5 44.9 44.0 respectively. Following latent class analysis (LCA), five distinct impairment patterns were noted, locomotor emerging as a central component across most classes. persons years included financial stability, being male or younger within cohort, junior high school higher education, married, no smoking history, manageable comorbidity levels, minimal medication use, good sleep, not using assistive devices.
