Introduction Emerging evidence indicates that children can be concurrently wasted and stunted (WaSt), increasing their mortality risk. However, more is needed to know about WaSt in Ethiopia. Therefore, this study aimed determine the trends predictors of using Ethiopian Demographic Health Survey datasets from 2000 2019. Methods The included a total weighted sample 34,930 aged 6–59 months. Descriptive multilevel mixed-effects (fixed random effects) logistic regression analyses were carried out. Intraclass Correlation Coefficient (ICC) Median Odds Ratio (MOR) calculated. Results prevalence was 1,682 (4.82%) with significantly decreasing trend, yielding percent change −57.51% (−69.37% −23.52%) In adjusted model, odds increased boys, shorter preceding birth interval, small size, delayed initiation complementary foods, diarrhea, fever, anemia, mother’s lack formal education, being farmer, poor/middle wealth index, mass media exposure. inversely related child’s age. Adjusted ICC MOR 31.16% 3.20%, respectively. Conclusion recommendations highlights importance considering individual community-level factors address WaSt, such as timely improving access health services, quality diet, prevention communicable diseases.
