Background The systemic immuno-inflammation index (SII), neutrophil-to-lymphocyte ratio (NLR), and platelet-to-lymphocyte (PLR) are widely used have been shown to be predictive indicators of various diseases. Diabetic nephropathy (DN), retinopathy (DR), peripheral neuropathy (DPN) the most prominent common microvascular complications, which seriously negative impacts on patients, families, society. Exploring associations with these three diabetic complications main purpose. Methods There were 1058 individuals type 2 diabetes mellitus (T2DM) in this retrospective cross-sectional study. SII, NLR, PLR calculated. diseases diagnosed by endocrinologists. Logistic regression subgroup analysis applied evaluate association between NLP, complications. Results significantly associated risk DN [odds ratios (ORs): 1.52, 1.71, 1.60, respectively] DR [ORs: 1.57, 1.79, 1.55, multivariate logistic regression. When NLR ≥2.66, OR was higher for DPN (OR: 1.985, 95% confidence interval: 1.29–3.05). Subgroup showed no significant positive across different demographics comorbidities, including sex, age, hypertension, HbA1c (glycated hemoglobin), dyslipidemia. Conclusion This study found a relationship DN, DR, DPN. In contrast, SII only DR. Therefore, diagnosis highly valuable.

Diabetic peripheral neuropathy (DPN) is a debilitating complication of diabetes mellitus, characterized by progressive nerve damage driven chronic hyperglycemia and systemic inflammation. The pathophysiology DPN significantly influenced pro-inflammatory cytokines, such as IL-1β, IL-6, TNF-α. These cytokines promote oxidative stress, vascular dysfunction, neuronal degeneration activating important signaling pathways including NF-κB MAPK. While IL-6 promotes microenvironment, increasing neuropathic pain, TNF-α IL-1β worsen Schwann cell failure compromising axonal support causing demyelination. Immune infiltration TLR activation increase the inflammatory cascade in DPN, resulting persistent neuroinflammatory state that sustains injury. main characteristics are degeneration, decreased neurotrophic support, which weaken transmission susceptibility to damage. Advanced glycation end-products, TNF-α, CXCL10 examples biomarkers may be used for early diagnosis disease progression monitoring. Additionally, crucial molecular targets have been found using proteomic transcriptome techniques, enabling precision medicine treatment DPN. This review emphasizes importance cytokine pathogenesis how cytokine-targeted treatments might reduce inflammation, restore function, improve clinical outcomes diabetic patients.

ABSTRACT Humans are perhaps evolutionarily engineered to get deeply addicted sugar, as it not only provides energy but also helps in storing fats, which survival during starvation. Additionally, sugars (glucose and fructose) stimulate the feel‐good factor, they trigger secretion of serotonin dopamine brain, associated with reward sensation, uplifting mood general. However, when consumed excess, contributes imbalance, weight gain, obesity, leading onset a complex metabolic disorder, generally referred diabetes. Type 2 diabetes mellitus (T2DM) is one most prevalent forms diabetes, nearly affecting all age groups. T2DM clinically diagnosed cardinal sign chronic hyperglycemia (excessive sugar blood). Chronic hyperglycemia, coupled dysfunctions pancreatic β‐cells, insulin resistance, immune inflammation, further exacerbate pathology T2DM. Uncontrolled T2DM, major public health concern, significantly toward progression several micro‐ macrovascular diseases, such diabetic retinopathy, nephropathy, neuropathy, atherosclerosis, cardiovascular including cancer. The current review discusses epidemiology, causative factors, pathophysiology, comorbidities, existing emerging therapies related It future roadmap for alternative drug discovery management

Diabetic neuropathy (DN) is a prevalent and painful complication of diabetes; however, the mechanisms underlying its pathogenesis remain unclear, effective clinical treatments are lacking. This study aims to explore role peroxisomes in Schwann cells DN. The abundance sciatic nerves mice or was analyzed using laser confocal super-resolution imaging western blotting. RFP-GFP-SKL (Ser-Lys-Leu) probe utilized assess pexophagy (peroxisomes autophagy) levels. To evaluate palmitoylation PEX11B, acyl-resin assisted capture (acyl-RAC) assay Acyl-Biotin Exchange (ABE) were employed.
