Background/Objectives: The socket-shield technique (SST) was developed to preserve the facial/buccal portion of a tooth root prevent post-extraction ridge resorption. It has gained attention for use in anterior implant sites, but its application posterior sites remains unexplored. aim this case report proof-of-principle using SST lower molar site and evaluate effectiveness preserving tissues. Methods: A 34-year-old non-smoking patient with non-restorable mandibular first (tooth #36) underwent immediate placement SST. tooth's crown removed, buccal segments roots were retained as "shields" while placed center socket. Preoperative postoperative cone-beam CT (CBCT) scans clinical exams used assess outcomes up 12 months. Results: procedure completed uneventfully. CBCT after 4 months showed minimal horizontal bone loss: ~0.2 mm at months; ~0.1 additional loss by peri-implant soft tissue profile remained stable, achieved osseointegration high primary secondary stability. Conclusions: In case, effectively preserved alveolar contours extraction site, avoiding collapse often seen post-extraction. This suggests may be viable preservation approach sites; however, long-term follow-up further studies are needed confirm sustained validate technique's predictability.
