Insomnia is highly prevalent among cancer survivors and can have serious implications if inadequately treated. Cognitive-behavioral therapy for (CBT-I) recommended as the first-line treatment insomnia but rarely available to survivors. We tested effectiveness of a web-based CBT-I program, OncoSleep, in Cancer with (n = 154) were randomly assigned (1:1) digital (6 weekly self-guided modules plus online clinician support) or waitlist control group. Patient-reported outcome measures severity (primary outcome), daytime functioning, sleep diaries administered at baseline post-treatment (8 weeks). Intention-to-treat analyses performed using mixed-effects models. Statistical tests two-sided. The group reported an average 11.0-point reduction Severity index (ISI), compared 1.4-point (p<.001). Statistically significant group-by-time interactions observed: produced significant, large effects improvements (d -2.56), cognitive functioning 0.95), physical 1.24) psychological quality life 0.80), fatigue -1.35). ABSTRACT Sleep Restriction Therapy (SRT) is a core component of cognitive behavioural therapy for insomnia (CBT‐I). Yet, adherence to SRT can be major challenge patients. As this evident in conventional face‐to‐face therapy, it raises the question internet‐delivered therapy. This qualitative study investigates CBT‐I and aims identify candidate variables that guide treatment decisions. Participants diagnosed with who participated an as part stepped‐care model were interviewed. Data from interviews 23 participants included study. A content analysis was employed. Hindering facilitating individual, intervention‐related, e‐coaching‐related factors examined. Moreover, consequences (e.g., negative effects attrition) Eleven consistently implemented SRT, two rejected beginning, eight discontinued, showed inconsistent adherence. Docility, low levels frustration tolerance, previous experiences emerged hindering individual factors. In contrast, high level insomnia‐related burden, social support, perceived fit program, being satisfied therapeutic guidance appeared foster Based on results study, path introduced, postulating predictors indication within docility SRT), engagement commitment support), potential process monitoring satisfaction need guidance).
