The Dietary Index for Gut Microbiota (DI-GM) is an emerging dietary indicator, its association with frailty has not been thoroughly investigated. Methods This study utilized data from NHANES 2007–2018 assessed the between DI-GM using multivariable weighted logistic regression, restricted cubic splines (RCS), subgroup analysis, mediation after adjusting relevant covariates. Results results indicate negative correlation frailty, each standard unit increase in reducing risk by 6% (OR = 0.940 [0.899, 0.984]). at different quartiles showed strong dose–response relationship, highest quartile showing 21.6% reduction. RCS analysis revealed linear relationship frailty.
