The narrative review aims to explore CRC pathogenesis by deciphering genetic-environmental interactions, analyzing the tumor microenvironment's role, and assessing treatment responses. These objectives seek enhance clinical decision-making improve patient care through a comprehensive understanding of disease. A from 2019 2024 on colorectal cancer (CRC) strategies was conducted. Systematic literature searches were performed PubMed, using CRC-related keywords ("Colorectal Neoplasms"[Mesh]) AND "Tumor Microenvironment"[Mesh]. Screening yielded 233 eligible studies, with 14 highly relevant ones included. PRISMA guidelines followed for transparency reproducibility. This spanning 2019-2024 shows diverse study designs (5 4 randomized controlled trials, 2 cohort 3 systematic reviews) varied sample sizes (from 4000 participants). Genetic mutations like KRAS BRAF are significant in pathogenesis, 8% exhibiting MMR proficiency. Immune cells paracrine signaling influential, therapeutic responses vary, limited efficacy reported certain combinations. highlights CRC's multifactorial nature complex dynamics. Abstract Cancer immunotherapy has sparked a wave of cancer research, driven by recent successful proof-of-concept clinical trials. However, barriers are emerging during its rapid development, including broad adverse effects, lack reliable biomarkers, tumor relapses, and drug resistance. Integration nanomedicine may ameliorate current immunotherapy. Ultra-large surface-to-volume ratio, extremely small size, easy modification surface nanoparticles enable them to selectively detect cells kill in vivo. Abstract Over the past few years, tumor microbiome is increasingly recognized for its multifaceted involvement in cancer initiation, progression, and metastasis. With application of 16S ribosomal ribonucleic acid (16S rRNA) sequencing, intratumoral microbiome, also referred to as tumor-intrinsic or tumor-resident has been found play a significant role microenvironment (TME). Understanding their complex functions critical identifying new therapeutic avenues improving treatment outcomes. This review first summarizes origins composition these microbial communities, emphasizing adapted diversity across diverse range types stages. Moreover, we outline general mechanisms by which specific microbes induce including activation carcinogenic pathways, deoxyribonucleic (DNA) damage, epigenetic modifications, chronic inflammation. We further propose may evade immunity promote angiogenesis support while uncovering influences on each step metastatic cascade, such invasion, circulation, seeding secondary sites. Additionally, closely associated with drug resistance efficacy modulating immune responses, metabolism, apoptotic pathways. Furthermore, explore innovative microbe-based strategies, engineered bacteria, oncolytic virotherapy, other modalities aimed at enhancing immunotherapeutic efficacy, paving way microbiome-centered frameworks.
