Abstract Background Despite the progress made in better understanding potential mechanisms of spinal manipulative therapy (SMT) and its treatment effects, a knowledge gap continues to exist when identifying specific factors that contribute perceived effect associated with SMT. The purpose study was explore perceptions chiropractic clinicians, interns, patients regarding what during doctor-patient encounter Methods This used convenience sampling enroll participants from teaching clinic United States. Semi-structured interviews were as main form data collection, which took place January-April 2024. subsequently analyzed using thematic analysis organized into themes through an iterative open coding process. Results Six rounds conducted for total 18 interviews. Each round consisted one patient who received including SMT, intern performed treatment, clinician oversaw treatment. After analyzing interview data, following five identified: Treatment Outcome, Therapeutic Alliance, Adjunctive Therapies, Significance Cavitation, Psychomotor Skills. theme multiple subthemes mentioned by participant groups at varying frequencies. Patients frequently importance improvement symptoms well good communication skills use adjunctive therapies. Interns valued functional change while clinicians focused on confidence levels psychomotor skills. There differing views significance cavitation, ranging indifference indication successful Conclusion qualitative identified several describe may In addition required perform educators practitioners should consider such therapeutic alliance between provider, therapies, assessment outcome intervention.

Abstract Background There is limited high-quality research examining conservative treatments for back-related leg pain (BRLP). This feasibility study was done in preparation a full-scale trial comparing whole-person supported self-management intervention to medical care chronic BRLP. Methods Participants were randomized 12 weeks of individualized delivered by physical therapists and chiropractors or consisting guideline-based pharmacologic care. Supported based on behavioral model that used whole person approach enhance participants capabilities, opportunities, motivations engage self-care. It combined BRLP education with psychosocial strategies (e.g., relaxed breathing, progressive muscle relaxation, guided imagery, communication skills) modalities such as exercise spinal manipulation therapy. Providers trained address participants’ needs use behavior change motivational techniques develop therapeutic alliance facilitate self-management. Feasibility assessed using pre-specified targets recruitment enrollment, delivery, data collection over the six-month period. In addition, areas potential refinement optimization processes protocols assessed. Results We met exceeded nearly all targets. Forty-two enrolled period 2022 very few individuals declined participation due preferences one treatment. All but participant received treatment 95% attended minimum number visits (self-management = 6, 2). At weeks, group reported engaging practices learned program 77% taking medications prescribed. Satisfaction high 85% reporting satisfaction overall. Self-management providers required activities at 72% visits. also noted some challenges navigating shared decision-making process deciding what tools prioritize. Over period, completion rates 91% monthly surveys 86% weekly surveys. Conclusion able demonstrate feasible identified important optimization.

Abstract Background Chronic low back pain (CLBP) presents a significant challenge for manual therapists. Recent advancements in research have highlighted the limitations of traditional biomedical and biopsychosocial models, prompting exploration alternatives. The biopsychosocial-enactive (BPS-E) model has emerged as promising alternative. This study aims to explore application BPS-E by therapists managing CLBP initiate meaningful dialogue about its use. Methods adhered Standards Reporting Qualitative Research. Guided constructivist grounded theory, we conducted semi-structured interviews with ten who are experts model. Data collection, conceptualization, analysis were systematically carried out identify key themes insights. Results core theme identified was “The person-centred approach,” three subthemes: “Opportunities implementing model”, “Utilizing Integrating Diverse Skills Holistic Care”, “Challenges model”. Conclusion provides insights into how incorporate their practice, demonstrating advantages over findings highlight need further training effectively implement clinical settings. begins an essential discussion on potential enhance care patients.

Introduction Treatment mechanisms are the underlying process or pathway through which a treatment influences body. This includes molecular, cellular and physiological processes pathways contributing to effect. Manual therapy (MT) evokes complex mechanistic responses across body systems, interacting with individual patient context promote response. Challenges arise as studies spread multiple professions, settings populations. The purpose of this review is summarize that have been reported occur MT application. Clinical Scenario : Sacroiliac joint dysfunction (SIJD) is pain in the sacroiliac region that can be debilitating and difficult to treat. Many interventions used treat SIJD, including exercise manual manipulation. The effectiveness of these treatments by themselves, or conjunction, not well understood. Question In active individuals with an intervention, manipulation, a combination 2 effective reducing improving function? Summary Key Findings Three studies were included this critically appraised topic. One study found both therapy manipulation decreased improved function, but no between-group differences. other therapy, decreasing function participants. Bottom Line results suggested therapies patients SIJD. While techniques alone may SIJD short term, more long term. Background: Musculoskeletal conditions with multisystem comorbidities present significant challenges in terms of prevention and rehabilitation. Despite advances instrumental technologies for diagnosis treatment, a gap remains between biomedical research practical application. Strengthening the therapeutic alliance involves improving patients’ understanding diagnostics while emphasizing face-to-face interactions. A team healthcare professionals, promoting an integrated approach combining manual movement therapies—such as physiotherapy, chiropractic, osteopathic care—can bridge this gap, well-being through person-centered framework. Emerging highlights role cognitive, neurophysiological, sociocultural factors body perception health, need inclusive, culturally sensitive care within practice. Introduction: Orthopedic manual physical therapy (OMPT) is renowned for its positive biophysical and psychosocial effects, making it an important tool in clinical practice. Although OMPT has long been a stable intervention used developed countries, adoption spiked developing including the Philippines. Despite this progress, no comprehensive study explored factors that may hinder or enable Filipino therapists (PTs) from utilizing into their daily The aim of was to conduct thorough behavioral analysis determine PTs' capabilities, opportunities, motivations using orthopedic practice, Theoretical Domains Framework (TDF) Behavioral Change Wheel model. Methods: This research cross-sectional survey employed validated TDF questionnaire covering various domains, knowledge, skills, social/professional role identity, beliefs about optimism, consequences, reinforcements, intentions, memory/attention decision processes, environmental context resources, social influences, emotions, regulation, goals. PTs were purposefully recruited participate in-person CE courses, conferences, symposiums, through media.
