Abstract: Recently, gene therapy has become a subject of considerable research and been widely evaluated in various disease models. Though it is considered as stand-alone agent for COVID-19 vaccination, still suffering from the following drawbacks during its translation bench to bedside: high sensitivity exogenous nucleic acids enzymatic degradation; severe side effects induced either by or components formulation; difficulty cross barriers before reaching therapeutic target. Therefore, successful application therapy, safe reliable transport vector urgently needed. Extracellular vesicles (EVs) are ideal candidate delivery drugs owing their low immunogenicity, good biocompatibility toxicity. Abstract Background Radiomic analysis of quantitative features extracted from segmented medical images can be used for predictive modeling prognosis in brain tumor patients. Manual segmentation the components is time-consuming and poses significant reproducibility issues. We compare prediction overall survival (OS) recurrent high-grade glioma(HGG) patients undergoing immunotherapy, using deep learning (DL) classification networks along with radiomic signatures derived manual convolutional neural (CNN) automated segmentation. Materials methods retrospectively retrieved 154 cases HGG multiple centers. Tumor was performed by expert radiologists a network (CNN). From tumors, 2553 were each case. A robust feature subset selected intraclass correlation coefficient between segmentations. The data divided into 9:1 ratio validated through ten-fold cross-validation tested on rotating test set. Features selection done Kruskal–Wallis test. Radiomics-based OS predictions, generated Support Vector Machine (SVM), compared two approaches against CNN model adapted classification. Model efficacy evaluated area under receiver operating characteristic curve (AUC). Results clinical AUC 0.640 ± 0.013 (mean 95% confidence interval) training set 0.610 0.131 radiomics based outperformed automatic (AUC 0.662 0.122 vs. 0.471 0.086, respectively) Robust improved performance to 0.700 0.102, 0.554 0.085. demonstrated promising results, an average 0.755 0.071 sets 0.101 Conclusion segmentation-derived predicting glioma immunotherapy. end-to-end similarly manual-segmentation-derived without need potential time-saving must weighed lower interpretability black box modeling.
