Abstract Background Tele-harm reduction (THR) is a telehealth-enhanced, peer-led, harm intervention delivered within trusted syringe services program (SSP) venue. The primary goal of THR to facilitate linkage care and rapid, enduring virologic suppression among people who inject drugs (PWID) with HIV. An SSP in Miami, Florida, developed circumvent pervasive stigma the traditional healthcare system. Methods During development, we conducted in-depth interviews PWID HIV ( n = 25) identify barriers facilitators via THR. We employed general inductive approach transcripts guided by iterative readings raw data derive concepts, themes, interpretations intervention. Results Of 25 interviewed, 15 were adherent medication; 4 but non-adherent; 6 not care. Themes that emerged from qualitative analysis included trust confidence have clinicians as opposed professionals Several treatment reported PWID, including perceived actual discrimination friends family, negative internalized behaviors, denial status, fear engaging Facilitators empathy respect staff, flexibility telehealth location, an overall destigmatizing approach. Conclusion identified receipt through Interviews helped inform centered on environment SSP.
