Atrial fibrillation (AF) is the most common major cardiac rhythm disorder in adults and causes considerable morbidity mortality, particularly elderly patients. We present case of a 75-year-old female admitted for acute heart failure with history AF recurrent exacerbations. Despite aggressive medical management amiodarone diuretics, patient continued to deteriorate. The cardiology team recommended electrical cardioversion re-establish normal sinus rhythm. By doing so, developed flash pulmonary edema after procedure required immediate intubation mechanical ventilation. There was need tracheostomy facilitate prolonged ventilatory support some time. Over time, they were weaned off ventilator discharged home full recovery. This illustrates challenges managing an possible risks due cardioversion.

Decreased left atrial appendage emptying velocity (LAAV) is a marker for thrombus formation. This study evaluates the association between LAAV and inflammatory indices in non-valvular fibrillation (AF) patients. The population was 1428 patients with AF, 875 of whom enrolled. Introduction and aim: Heart failure with preserved ejection fraction (HFpEF) is a significant clinical challenge, often coexisting atrial fibrillation (AF), which exacerbates patient outcomes by increasing risks of stroke, hospitalizations, mortality. Recent studies suggest that epicardial adipose tissue (EAT), metabolically active fat depot, may contribute to AF pathogenesis promoting remodeling fibrosis. This study aimed evaluate the relationship between EAT thickness in HFpEF patients. Materials methods: A total 110 patients were included, 20 (18.2%) having documented AF. was measured using transthoracic echocardiography, confirmed via electrocardiography. Results: Patients had significantly greater compared those without (8.3 ± 0.9 mm vs. 7.1 0.8 mm, p < 0.001). Receiver operating characteristic (ROC) analysis demonstrated strong predictor (AUC = 0.87, 0.001), cut-off value 7.5 achieving 89% sensitivity 75% specificity. Conclusion: These findings indicate increased independently associated patients, highlighting its potential as biomarker for risk stratification. Future should explore whether targeting could improve this high-risk population.
