We evaluated the efficacy mental health apps on sought identify characteristics associated with effect size estimates. Sixty-nine randomised controlled trials (RCTs) were included. Random effects meta-analyses performed putative moderators examined at univariate multivariate (combinations interactions) levels. From 78 comparisons, we observed a small but significant pooled over control conditions perceived levels (g = 0.27; 95% CI 0.20, 0.34; I2 68%). This weakened after taking into account small-study bias according trim-and-fill procedure 0.10; 0.02, 0.19; 78%). Delivery monitoring features produced smaller estimates, although this association interacted other trial (small sample inactive group) analyses, suggesting that may have been explained by characteristic low-quality trials. Mental appear small, acute reducing stress. Future research should shift focus towards identifying change mechanisms, longitudinal outcomes, facilitate sustained app usage, tangible pathways integrating real-world clinical settings.

Participants predominantly 12 25 representation diverse identities, including Black, Asian, Hispanic, mixed race, Aboriginal individuals. Engagement varied, culturally relatable, low-cost showing higher retention user satisfaction. Linguistic country origin impeded some interventions, while near-peer mentorship, coproduction, tailored content improved DMHIs. While initial results are promising, small sample sizes, heterogeneity outcome assessments, a paucity longitudinal data robust comparisons generalizability.
