ABSTRACT Background Feasibility and effectiveness of online pain management programs for chronic widespread conditions have been shown; yet, there are no empirically evaluated hypermobile Ehlers–Danlos syndrome (hEDS) or hypermobility spectrum disorder (HSD). Bridging this gap, the Hypermobile Online Pain managemEnt (HOPE) program was developed with stakeholder input to be pilot tested. Methods We conducted a randomised controlled trial HOPE assess Intervention Measure (FIM), Acceptability (AIM) Appropriateness (IAM) eight clinical outcomes. participants were given 8 weeks complete 12 self‐paced modules, while control continued treatment as usual. Participants assessed at baseline, post‐treatment 3 months using surveys. Results Seventy‐two recruited randomised. Two withdrew before starting, leaving 34 controls 36 intervention analysis. In all, 81%–91% agreed/completely agreed that feasible mean FIM score 4.3/5 (SD 0.7), 62%–76% it acceptable AIM 3.9/5 0.96) 67%–76% appropriate IAM 4.0/5 0.9). Only two outcomes significantly improved in group, moderate small effect sizes worst intensity (Cohen's d = 0.63) impact 0.32) post‐intervention, respectively. Conclusion The seemed feasible, appropriate, preliminary improvements hypermobility. Our findings warrant fully powered further explore intervention. Significance Statement is high concern among people (HSD), yet limited resources them. This novel important guiding creation evidence‐based stakeholder‐relevant resources. Promising results suggest importance refinement retesting its wider implementation hEDS/HSD population so empower them self‐management.
