Introduction. Dermatological manifestations of a novel coronavirus infection caused by the severe acute respiratory syndrome 2 (SARS-CoV-2) are observed in different periods disease, and can sometimes be only manifestation disease 2019 (COVID-19). Despite fact that at present daily increase new cases Russian Federation is small, issues timely diagnosis, treatment prevention COVID-19 remain relevant, experience gained during pandemic invaluable practical importance. Aim. Evaluation dermatological associated with SARS-CoV-2 period post-acute one. Materials methods. A cohort non-comparative descriptive study 120 people aged 48–72 years (44 men, 76 women), who had no earlier than 12 weeks ago, was carried out. Results. Changes skin its appendages were 72 (60%) patients. In 7 (9.7%) people, symptoms appeared represented urticaria, pernio-like lesions feet, edematous erythema face chest, itching hyperesthesia skin. two cases, rashes preceded onset signs infection. post-COVID occurred 94.4% patients, which diffuse alopecia diagnosed 60.3% xerosis and/or paresthesia 48.5%, onychopathy 23.5%, new-onset dermatoses 14.7%, 38.2% – combination several disorders. Conclusion. Among COVID-19, we identify typical characteristic syndrome. The serve as an indicator infection, contributing to diagnosis. Knowledge will allow us correctly conduct diagnostic search prescribe adequate treatment.

Мета роботи – на підставі клініко-морфологічного дослідження особливостей уражень легень і шлунково-кишкового тракту різних стадіях запального процесу обґрунтувати гіпотезу ендотоксикозу як ключового патогенетичного фактора системної ендотеліальної дисфункції вторинного ураження при COVID-19. Матеріали методи. Дослідження базується клініко-патоморфологічному вивченні 52 випадків смерті від COVID-19, з яких для ретельного гістологічного був відібранний аутопсійний матеріал 18 померлих. У частині задіяний імерсійний метод мікроскопії, виконували імуногістохімічне дослідження. Результати. Основним проявом альтеративного розвитку дифузного альвеолярного пошкодження (ДАП) мікротромбоз фоні лейкоцитарних пасток, що формувались нетозом гіперактивованих поліморфноядерних лейкоцитів. Встановили відсутність ознак попереднього міграції лейкоцитів через стінки капілярів у просвіт альвеол. Одночасно гістологічна картина змін в тонкій кишці відповідала гострому десквамативно-ерозивному запальному вірусно-бактерійної етіології ознаками активної транслокації ендотоксину. В інфільтраті слизової оболонки тонкої кишки гіперергічні вогнищеві лімфоїдні інфільтрати, стадії ранньої ексудації (p<0,005), так проліферації ДАП (p<0,001). Продемонстровано достовірно низький показник імунорегуляторного індексу (ІРІ) CD4/CD8 гістологічних зразках інфільтрату порівняно ІРІ Запропоновано модель патогенезу згідно якою може виникати вторинно, за механізмом гепатопульмонального синдрому. Висновки. Є підстави вважати, вхідними воротами збудника тяжкому перебігу поряд дихальною системою, є слизова оболонка дистальних відділів кишки. Виявлені морфологічні прояви тяжкої пошкодженням ендотелію свідчать більше про ендотоксином параалергічної реакції, а не пряме вірусне ураження.
