Elevated levels of homocysteine (Hcy) and related metabolites are associated with Alzheimer's disease (AD). Severe hyperhomocysteinemia causes neurological deficits worsens behavioral biochemical traits AD. Patients with sepsis-associated delirium (SAD) show severe neurological impairment, often require an intensive care unit (ICU) stay and have a high risk of mortality. Hence, useful biomarkers for early detection SAD are urgently needed. Extracellular vesicles (EVs) their cargo known to maintain normal physiology but also been linked numerous disease states. Here, we sought identify differentially expressed proteins in plasma EVs from patients as potential SAD. Plasma 11 age-matched septic without (non-SAD) were isolated by differential centrifugation, characterized nanoparticle tracking analysis, transmission electron microscopy Western blot analysis. Differential EV protein expression was determined mass spectrometry the resulting proteomes Gene Ontology term between-group statistics. As preliminary results because small group size, five distinct showed significantly different pattern between non-SAD (p ≤ 0.05). In patients, upregulated included paraoxonase-1 (PON1), thrombospondin 1 (THBS1), full fibrinogen gamma chain (FGG), whereas downregulated comprised immunoglobulin (IgHV3) complement subcomponent (C1QC). Thus, significant changes involved immune system regulation blood coagulation well lipid metabolism this pilot study. They might be indicator pathogenesis thus warrant further examination biomarkers, research is needed expand on these findings longitudinal study designs larger samples comprehensive polymodal data collection.
