Abstract Type 2 diabetes mellitus (T2DM) can lead to dysfunction in the brain's glymphatic system, but relationship between system function and insulin resistance remains unclear. The aim of this study was investigate connection changes T2DM surrogate marker for resistance, Triglyceride-Glucose Index (TyG). A total 28 patients (T2DM group) 24 healthy volunteers (HC were included study. We applied an MRI technique diffusion tensor imaging along the perivascular space (DTI-ALPS) for assessing glymphatic system (GS) in a genome-wide association study (GWAS) and phenome-wide (PheWAS) of 40,486 European individuals. Exploratory analysis revealed 17 genetic loci significantly associating with regional DTI-ALPS index. found 58 genes, including SPPL2C EFCAB5 , which prioritized index subtypes associated neurodegenerative diseases. PheWAS 241 traits suggested that body mass blood pressure phenotypes closely related to GS function. Moreover, we detected disrupted function 44 625 predefined disease conditions. Notably, Mendelian randomization mediation indicated lower was risk factor ischemic stroke (odds ratio = 1.56, P 0.028) by partly mediating obesity. Results provide insights into architecture mechanism DIT-ALPS highlight its great clinical value, especially cerebral stroke.

Objective To investigate the efficacy of diffusion tensor imaging (DTI) in assessing motor impairment resulting from minor basal ganglia hemorrhage post-stereotactic surgery. Methods A total 104 patients with hemorrhages (hematoma volume ≤15 ml) underwent DTI within 48 h and 1 month post-treatment. Patients were divided into two groups: 42 experimental group, receiving stereotactic surgery medication, 62 control medication alone. The corticospinal tract (CST) posterior limb internal capsule was region interest (ROI) for mean FA values on both sides. Fugl-Meyer function (FMF) scores recorded post-treatment, Modified Rankin Scale (MRS) at 6 months. Background and Purpose This study aimed to investigate early changes in interstitial fluid (ISF) flow patients with severe carotid stenosis after angioplasty stenting (CAS).Methods We prospectively recruited participants ≥80% undergoing CAS at our institute between October 2019 March 2023. Magnetic resonance imaging (MRI), including diffusion tensor (DTI), the Mini-Mental State Examination (MMSE) were performed 3 days before CAS. MRI DTI MMSE conducted within 24 hours 2 months CAS, respectively. The image analysis along perivascular space (DTI-ALPS) index was calculated from data determine ISF status. Increments defined as ratio of difference post- preprocedural values values.Results In total, 102 (age: 67.1±8.9 years; stenosis: 89.5%±5.7%) longitudinal evaluated. DTI-ALPS increased (0.85±0.15; 0.85 [0.22] vs. 0.86±0.14; 0.86 [0.21]; <i>P</i>=0.022), did score (25.9±3.7; 24.0 [4.0] 26.9±3.4; 26.0 [3.0]; <i>P</i><0.001).
