Background The Oxidative Balance Score (OBS), which quantifies the balance between antioxidants and pro-oxidants influenced by diet lifestyle, is crucial given oxidative stress’s significant role in Chronic Kidney Disease (CKD). This study aims to determine association OBS CKD using data from National Health Nutrition Examination Survey (NHANES) 1999-2018. Methods We analyzed spanning 1999 2018. was constructed a detailed array of 20 factors, including dietary nutrients lifestyle behaviors. relationship risk evaluated weighted logistic regression models, adjusted for potential confounders, with generalized additive model (GAM) examining non-linear associations. Subgroup analyses interaction effects across diverse demographic clinical groups, along sensitivity analyses, were performed validate findings. Results Among 32,120 participants analyzed, 4,786 identified CKD. Fully analysis revealed that each unit increase associated 2% reduction prevalence [OR: 0.98 (0.98–0.99), P < 0.001]. The two forms of Klotho (membrane-bound soluble form) have diverse pharmacological functions such anti-inflammatory, anti-oxidative, anti-fibrotic, tumour-suppressive etc. membrane-bound form plays a pivotal role maintaining kidney homeostasis by regulating fibroblast growth factor 23 (FGF 23) signalling, vitamin D metabolism phosphate balance. deficiency has been linked with significantly reduced protection against various pathological phenotypes, including diabetic disease (DKD), which major cause chronic leading to end-stage disease. Owing pleiotropic actions klotho, it shown beneficial effects DKD tackling complex pathophysiology reducing inflammation, oxidative stress, well fibrosis. Abstract To explore the optimal cut-off values of Klotho for predicting all-cause and cardiovascular mortality among chronic kidney disease (CKD) patients. was measured in 40–79-year-old individuals NHANES 2007–2016. A total 2418 patients with stage 1–4 CKD were included. The utilized using receiver operator characteristic (ROC) curves be verified on effects mortality. Restricted cubic splines used to examine relationship between cutpoints as reference. After a mean follow-up period 87.9 months, 535 deaths occurred 188 died disease. Cubic showed that risk increased gradually < 700 pg/ml. ROC revealed are 548.8 pg/ml 660.9 pg/ml, respectively. Compared higher levels Klotho, HRs (95% CIs) 1.52 (1.23, 1.87) 1.58 (1.13, 2.22) lower respectively, multivariate model ( P .0001 = 0.008). Our findings CKD.
