Background: Aging is a key risk factor for atherosclerosis progression that associated with increased incidence of ischemic events in supplied organs, including stroke, coronary events, limb ischemia, or renal failure. Cardiovascular disease the leading cause death and major disability adults ≥ 75 years age. Atherosclerotic occlusive affects everyday activity, quality life, it reduced life expectancy. As most multicenter randomized trials exclude elderly very patients, particularly those severe comorbidities, physical cognitive dysfunctions, frailty, residence nursing home, there insufficient data on management older patients presenting atherosclerotic lesions outside territory. This results serious critical gaps knowledge lack guidance appropriate medical treatment. In addition, due to variety comorbidities elderly, average daily number pills taken by octogenarians exceeds nine. Polypharmacy frequently drug therapy problems related interactions, toxicity, falls injury, delirium, non-adherence. Therefore, we have attempted gather treatment extra-cardiac indicating where some evidence are evidence-based medicine. Public PubMed databases were searched review existing effectiveness lipid-lowering, antithrombotic, new glucose-lowering medications disease.

Atherosclerosis is a multifactorial disease of medium and large arteries, characterized by the presence lipid-rich plaques lining intima over time. It main cause cardiovascular diseases death worldwide. Redox imbalance lipid peroxidation could play key roles in atherosclerosis promoting bundle responses, including endothelial activation, inflammation, foam cell formation. The oxidation polyunsaturated fatty acids generates various products such as reactive carbonyl species (RCS), 4-hydroxy alkenals, malondialdehyde, acrolein. RCS covalently bind to nucleophilic groups nucleic acids, phospholipids, proteins, modifying their structure activity leading progressive dysfunction. Protein lipoxidation non-enzymatic post-translational modification proteins RCS. Low-density lipoprotein (LDL) apolipoprotein B (apoB) major role Moreover, oxidized LDLs are source RCS, which form adducts on huge number depending oxidative stress intensity, nature targets, availability detoxifying systems. Many systems affected lipoxidation, extracellular matrix components, membranes, cytoplasmic cytoskeletal transcription factors, other components. mechanisms involved lipoxidation-induced vascular dysfunction not fully elucidated. In this review, we focus protein during atherogenesis.

The market for dietary supplements has significantly grown due to their envisaged health benefits. These bioactive compounds are actively explored therapeutic benefits against vast metabolic complications abundant antioxidant properties. Nuclear factor erythroid 2-related 2 (Nrf2) modulation as a cytoprotective mechanism oxidative stress-induced stress garnered interest, particularly through its regulation by many supplements. This review aims evaluate and critically discuss the available evidence on potential effects of supplements, including prominently used nutraceuticals dyslipidemia-associated cardiovascular complications. Notably, this highlights these modulate Nrf2 pathway, enhancing intracellular antioxidants providing protection Beyond value preclinical ability regulate pathway counteract damage, also incorporates clinical supporting in dyslipidemia-related conditions. Fundamental pharmacological aspects such bioavailability relevant biological properties discussed inform further development diseases.

Aims/Background Rheumatoid arthritis (RA) is an inflammatory autoimmune disease and N-acetylcysteine (NAC) considered a potential therapeutic agent for RA due to strong antioxidant anti-inflammatory properties. Therefore, this systematic review meta-analysis aimed evaluate the efficacy of NAC as adjuvant therapy RA. Methods A search was conducted across five databases from inception 1 August 2024, including CINAHL, Cochrane Library, EMBASE, PubMed, Web Science. The risk-of-bias tool randomized trials used assess quality included studies. Sensitivity analysis performed when significant heterogeneity identified. Results Four studies involving 204 patients were in our meta-analysis. results indicated that alleviated activity (Disease Activity Score 28-erythrocyte sedimentation rate (DAS28-ESR): mean difference (MD) = 0.54). Additionally, reduced markers (erythrocyte (ESR): MD 3.00). However, beneficial effects on oxidative stress not observed. Cardiovascular diseases (CVDs) continue to be the leading cause of mortality worldwide, necessitating development novel therapies. Despite therapeutic advancements, underlying mechanisms remain elusive. Reactive oxygen species (ROS) show detrimental effects at high concentrations but act as essential signalling molecules physiological levels, playing a critical role in pathophysiology CVD. However, link between pathologically elevated ROS and CVDs pathogenesis remains poorly understood. Recent research has highlighted remodelling epigenetic landscape crucial factor CVD pathologies. Epigenetic changes encompass alterations DNA methylation, post-translational histone modifications, adenosine triphosphate (ATP)-dependent chromatin noncoding RNA transcripts. Unravelling intricate is challenging due complexity signals gene regulation. This review aims provide insights into modulating within cardiovascular system. Understanding these interactions may offer strategies for managing by targeting ROS-induced changes.
