Dedifferentiated liposarcoma (DDLP) was initially defined as a tumor containing differentiated and distinct regions of nonlipogenic spindle cell or pleomorphic sarcoma. Retroperitoneal liposarcomas feature characteristic appearance with predominantly fatty component, cystic are rare. We describe case retroperitoneal DDLP consisting multilocular cysts.A 77-year-old man previously visited doctor because an echo scan unexpectedly revealed abdominal tumor. Contrast computed tomography (CT) disclosed large spanning from the left upper abdomen to retroperitoneum, poorly marginated soft tissue structures were present around aorta, inferior vena cava, pancreas, mesentery, kidney. CT also right lung mass. The in mass strongly enhanced on 2-deoxy-2-[fluorine-18] fluoro-d-glucose positron emission tomography, suggesting malignant metastasis. CT-guided percutaneous biopsy targeting perirenal structure performed, diagnosed DDLP. Lung metastasis present, surrounded multiple organs. Therefore, not suitable for surgical resection but it indicated chemotherapy based multidisciplinary discussion.We experienced by image-guided treated appropriately pathological diagnosis.
