The standard oncologic treatment of locally advanced rectal cancer is long-course radio-chemotherapy followed by surgery and adjuvant chemotherapy. This can result in a lengthy total duration, sometimes up to one year from the diagnosis. Interruptions neoadjuvant occur for variety reasons, forced or unforced. main purpose this study analyze survival data patients who received find cut-off point showing exactly how many days interruption risk death disease relapse increases. We conducted retrospective on 299 with using analysis (Kaplan–Meier curve Cox regression) determine probabilities overall survival, local control, disease-free survival. Patients 0 3 therapy had higher probability compared 4 more (90.2% 57.9%, p-value < 0.001), hazard ratio 5.89 (p 0.001). Local control 0–2 people (94% vs. 75.4%, 82.2% 50.5%, respectively, both p-values tumoral nodal downstaging experienced fewer than no downstage. These findings reinforce need radiation oncologists be well-organized when starting cancer, order anticipate prevent potential interruptions achieve best therapeutic results.

Background/Objectives: In Romania, breast cancer is the second most common cancer, third leading cause of death, and prevalent overall. De novo advanced-stage often presents in clinical practice, treatment decisions are best made a multidisciplinary tumor board (MTD) involving surgeons, radiotherapists, medical oncologists. Significant advances systemic therapies, particularly progression-free survival (PFS) overall (OS), have surpassed traditional palliative mastectomy radiotherapy for local control. Therefore, purpose this study to emphasize importance initial choice patient prognosis. Methods: We expose two cases patients with de severe, advanced-stage, hormone receptor (HR)-positive, human epidermal growth factor 2 (HER2)-negative their management outcome using cyclin-dependent kinase (CDK) 4/6 inhibitor radiotherapy. An extensive review literature from past five years was also conducted. Results: The role diminishing, as many opting novel including inhibitors, which may improve quality life. Conclusions: First-line therapy locally advanced has suffered changes due implementation targeted therapy. However, drug resistance—either or acquired—remains critical consideration. MTD discussions informed essential achieving personalized, evidence-based outcome.

Background and clinical significance: Phyllodes tumors (PTs) are rare stromal neoplasms originating in the connective tissue of breast, distinct from carcinomas that arise ducts or lobules. These exhibit a broad spectrum morphologic features traditionally classified as benign, borderline, malignant. Case presentation: We present case 71-year-old female diagnosed with malignant PT treated at our hospital. The patient noticed gradually enlarging lump her right breast over several months. Mammography was inconclusive, but an ultrasound later revealed lobulated, firm mass, BIRADS 5. Physical examination identified 20 cm core needle biopsy suggested borderline PT. Following lumpectomy, pathology confirmed tumor narrow surgical margins (0.1 cm). Although mastectomy recommended to achieve wider margins, opted for adjuvant radiotherapy. She received 50 Gy 25 fractions whole followed by 16 boost bed 8 fractions. treatment well tolerated completed successfully. Initially, patient’s therapeutic management delayed due combination personal organizational factors. However, process streamlined through use novel digital tool developed facilitate entire journey within hospital system. Conclusions: This highlights diagnostic complexities PTs, critical need effective collaboration between specialties, importance timely planning optimal outcomes.

Introduction and Clinical Significance: Lung cancer, a leading cause of global cancer diagnoses, maintains the highest mortality risk despite advances in treatment. Immunotherapy agents, such as anti-programmed death-1/programmed death ligand-1 (PD-1/PD-L1), have revolutionized care for non-small cell lung (NSCLC). However, success is tempered by emergence immune-mediated adverse reactions, including rare onset type I diabetes. The incidence diabetes mellitus increased during SARS-CoV-2 pandemic. While there are several cases new-onset after COVID-19 vaccination, no case 1 was described an immune checkpoint inhibitor (ICI)-treated patient. Case Presentation: A 57-year-old male with stage IV NSCLC (brain liver metastases) who had been treated nivolumab 4 years appeared positive infection at routine check. After two weeks, he admitted to our clinic severe fatigue, hyperglycemia, hyponatremia, hyperkalemia. HbA1c level normal serum peptide C undetectable. Nivolumab treatment ceased, patient became fully dependent on basal–bolus insulin. 3 months, showed complete imagistic remission. Conclusions: presented significant challenges due unclear etiology newly uncommon age which developed. outcome suggests that anti-PD-1 can act synergistically.
