Acute ischemic stroke (AIS) remains a major cause of mortality and long-term disability worldwide, driven by complex multifaceted etiological factors. Metabolic dysregulation, gastrointestinal microbiome alterations, systemic inflammation are emerging as significant contributors to AIS pathogenesis. This review addresses the critical need understand how these factors interact influence risk outcomes. We aim elucidate roles dysregulated adipokines in obesity, impact gut microbiota disruptions, neuroinflammatory cascade initiated lipopolysaccharides (LPS) AIS.
