Abstract Background Suboptimal health status (SHS) is a reversible borderline condition between optimal and diseases. Although the definition of SHS widely understood, questionnaires are needed to be further developed, by which individual with can identified from variety population in context predictive, preventive personalized medicine (PPPM/3PM). This study aimed develop short form suboptimal questionnaire (SHSQ-SF) reference questionnaire-25 (SHSQ-25). Methods A total 6,183 participants enrolled southern China were included training set, while 4,113 northern an external validation set. SHSQ-SF nine key items SHSQ-25, that has been applied Caucasians, Asians, Africans. Item analysis, reliability validity tests carried out validate SHSQ-SF. Receiver operating characteristic (ROC) curve was used identify cutoff value for diagnosis SHS. Results In dataset, Cronbach’s α coefficient 0.902, split-half 0.863. The Kaiser-Meyer-Olkin (KMO) statistic 0.880, Bartlett’s test sphericity significant (χ 2 = 32,929.680, P< 0.05). Both Kaiser’s criteria (eigenvalues >1) scree plot revealed one factor explained 57.008% variance. Standardized loadings confirmatory analysis (CFA) indices 0.59 0.74, χ / dƒ 4.972, (GFI) 0.996, CFI RFI 0.989 RMSEA 0.031. area under ROC (AUC) 0.985 (95%CI: 0.983 – 0.988) (≥ 11) this showed good discriminatory power (AUC 0.975, 95%CI: 0.971 0.979), sensitivity 96.2% specificity 87.4%.
