Optimal nutrient intake ensuring better neurodevelopment for very low birth weight (VLBW) infants remains unknown. Abstract Extubation failure (EF) is a significant concern in mechanically ventilated newborns, and predicting its occurrence an ongoing area of research. To investigate the predictors EF newborns undergoing planned extubation, we conducted systematic review meta-analysis. A literature search was PubMed, Web Science, Embase, Cochrane Library for studies published English from inception each database to March 2023. The PRISMA guidelines were followed all phases this review. Risk Bias Assessment Nonrandomized Studies tool used assess methodological quality. Thirty-four included, 10 which overall low risk bias, 15 moderate 9 high bias. reported 43 possible six broad categories (intrinsic factors; maternal diseases adverse conditions newborn; treatment characteristics before after extubation; clinical scores composite indicators). Through qualitative synthesis quantitative meta-analysis 19 factors, identified five definite eight 22 unclear factors related EF. Definite included gestational age, sepsis, pre-extubation pH, FiO 2 , respiratory severity score.
