Over three years’ pandemic of 2019 novel coronavirus disease (COVID-19), multiple variants and subvariants have emerged successively, outcompeted earlier become predominant. The sequential emergence reflects the evolutionary process mutation-selection-adaption severe acute respiratory syndrome 2 (SARS-CoV-2). Amino acid substitution/insertion/deletion in spike protein causes altered viral antigenicity, transmissibility, pathogenicity SARS-CoV-2. Early pandemic, D614G mutation conferred virus with advantages over previous increased it also laid a conservative background for subsequent substantial mutations. role genomic recombination evolution SARS-CoV-2 raised increasing concern occurrence recombinants such as Deltacron, XBB.1.5, XBB.1.9.1, XBB.1.16 late phase pandemic. Co-circulation different co-infection immunocompromised patients accelerate recombinants. Surveillance variations, particularly recombination, is essential to identify ongoing changes genome antigenic epitopes thus leads development new vaccine strategies interventions.

Neutralising antibodies against the SARS-CoV-2 spike (S) protein are major determinants of protective immunity, though insufficient antibody responses may cause emergence escape mutants. We studied humoral immune response causing intra-host evolution in a B-cell depleted, haemato-oncologic patient experiencing clinically severe, prolonged infection with virus lineage B.1.177.81. Following bamlanivimab treatment at an early stage infection, developed bamlanivimab-resistant mutation, S:S494P. After five weeks apparent genetic stability, additional substitutions and deletions within N-terminal domain (NTD) receptor binding (RBD) S was observed. Notably, composition frequency mutations changed short period unprecedented dynamic. The triple mutant S:Delta141-4 E484K S494P became dominant until elimination. Routine serology revealed no evidence patient. A detailed analysis variant-specific by pseudotyped neutralisation test, surrogate immunoglobulin-capture enzyme immunoassay showed that onset IgM-dominated coincided appearance mutations. formation neutralising correlated One year later, experienced mild re-infection Omicron BA.1.18, which treated sotrovimab resulted increase Omicron-reactive antibodies. In conclusion, endogenous immunocompromised NTD RBD virus. Although elimination ultimately achieved, this escaped detection routine diagnosis created situation temporarily favouring rapid various mutants known epidemiological relevance.

The second wave of COVID-19 occurred in South America early 2021 and was mainly driven by Gamma Lambda variants. In this study, we aimed to describe the emergence local genomic diversity SARS-CoV-2 variant Argentina, from its initial entry into country until detection ceased. Molecular surveillance conducted on 9356 samples Argentina between October 2020 April 2022, sequencing, phylogenetic, phylogeographic analyses were performed. Our findings revealed that first detected January steadily increased frequency it peaked 2021, with continued throughout year. Phylodynamic showed at least 18 introductions occurred, nine them having evidence onward transmission. spatial–-temporal reconstruction Argentine clades associated sequences Latin suggested an diversification Metropolitan Area Buenos Aires before spreading other regions Argentina. Genetic genome allowed us mutational patterns detect rare mutations immunocompromised patient. study highlights importance identifying introduction geographical distribution variant, as well monitoring could be involved evolutionary leaps characterize variants concern.
