Esophageal lichen planus (ELP) is a rare chronic inflammatory condition with delayed diagnosis that can present dysphagia. It has been scarcely studied in the literature, and clinical management guidelines are lacking. Stenosis neoplastic transformation among its most feared complications. We fifth reported case of ELP to be successfully treated Upadacitinib.

As of now, there exists no established therapy for ELP. Retinoids, which are standard in treating cutaneous LP, do not exhibit positive effects While topical glucocorticosteroids often yield favorable responses esophageal inflammation, some cases prove recalcitrant or refractory. In such instances, various immunosuppressive therapies have been attempted with variable success. This report details a severe case ELP that showed resistance to prednisolone, acitretin, alitretinoin, adalimumab, tacrolimus, hydroxychloroquine plus mycophenolate mofetil, and cyclophosphamide. The initiation the JAK inhibitor tofacitinib induced an impressive clinical, endoscopic, histological remission. This response is discussed context our evolving understanding immune-mediated pathogenesis this disease.

Esophageal squamous cell carcinoma (ESCC) remains the most common malignancy of esophagus worldwide. Environmental and lifestyle exposures such as alcohol tobacco have been well defined in pathogenesis ESCC, acting concert with intrinsic epigenomic, genomic transcriptomic changes. However, a variety nonenvironmental etiologies including Fanconi anemia, lichen planus, chronic mucocutaneous candidiasis, esophageal epidermoid metaplasia, epidermolysis bullosa, tylosis, atresia, achalasia receive minimal attention despite high risk ESCC these diseases. The goal this review was to promote clinical recognition suggest diagnostic framework for earlier detection patients rare In all discussed conditions, change symptoms should trigger prompt endoscopic evaluation, surveillance programs advanced imaging techniques chromoendoscopy be considered. Moreover, we leverage convergence diseases on identify mechanisms underlying malignant transformation aberrant proliferation, mucosal barrier dysfunction, increased inflammation, genome instability. study, summarize presentation, pathologic findings, potential screening strategies, associated that drive ESCC.

Lymphocyte-rich inflammation of the esophageal mucosa has gained increased awareness among pathologists and clinicians recently. Patients usually present with symptoms dysfunction, including dysphagia food bolus impaction. Endoscopy may show changes similar to eosinophilic esophagitis but also be entirely normal ('microscopic esophagitis'). Three morphological subtypes or variant forms have been described which include lymphocytic, lichenoid lymphocyte-predominant esophagitis. These need discriminated against other distinct causes lymphocytosis, such as gastro-esophageal reflux disease Candida infection.
