During transoral endoscopic thyroidectomy, preserving the recurrent laryngeal nerve (RLN) is a major challenge because visualization of this often obstructed by thyroid itself, increasing risk serious complications. This study explores application an augmented reality (AR) system to facilitate easier identification RLN during thyroidectomy. Three patients scheduled for thyroidectomy were enrolled in proof-of-concept study. Preoperative computed tomography scans used create AR model that included thyroid, trachea, veins, arteries, and RLN. The was overlaid onto real-time camera images live surgeries. Manual registration performed using customized controller. aligned with surgical landmarks such as trachea common carotid artery. accuracy assessed Dice similarity coefficient (DSC) evaluate alignment between real model. 3 female (mean age, 33.3 ± 15.7 years), mean tumor size 1.0 0.3 cm. All underwent right lobe. Final histopathological diagnoses comprised 2 papillary carcinomas one follicular adenoma. manual 0.60, 0.70, 0.57 1, 2, 3, respectively, value 0.6 0.1. proved feasible demonstrated potential improving localization anatomical structures, particularly RLN, indicated moderate DSC.

Active surveillance (AS) has been widely adopted as an alternative to immediate surgery owing the indolent nature and favorable outcomes of papillary thyroid microcarcinoma (PTMC). AS is generally recommended for tumors measuring ≤1 cm without aggressive cytological subtypes, risk gross extrathyroidal extension (ETE), lymph node metastasis (LNM), or distant metastasis. requires careful patient selection based on various tumor characteristics, ultrasound (US) findings. Moreover, during AS, regular US performed monitor any signs progression, including growth, new features potential ETE, LNM. Therefore, appropriate imaging-based assessment plays a crucial role in determining whether should be pursued. However, detailed recommendations concerning evaluation are currently insufficient, necessitating formulation this guideline. The Korean Society Thyroid Radiology developed consensus statement low-risk PTMC, covering methods when considering management option conducting follow-up imaging tests AS. This guideline aims provide optimal scientific evidence expert opinion regarding standardized US-based protocol PTMC.

Papillary thyroid carcinoma (PTC) is the most common malignancy, characterized by a range of subtypes that differ in their cytologic features, clinical behavior, and prognosis. Accurate evaluation PTC using fine-needle aspiration essential but can be challenging due to morphologic diversity among subtypes. This review focuses on distinct characteristics various subtypes, including classic type, follicular variant, tall cell, columnar hobnail, diffuse sclerosing, Warthin-like, solid/trabecular, oncocytic PTCs. Each subtype demonstrates unique nuclear architectural patterns, background elements for diagnosis differentiation from other lesions. Recognizing these patterns identifying aggressive like cell PTCs, which have higher risk recurrence, metastasis, poorer outcomes. Additionally, rare such as sclerosing Warthin-like PTCs present profiles must carefully interpreted avoid diagnostic errors. The also highlights indicators lymph node metastasis high-grade differentiated carcinoma. integration molecular testing further refine specific genetic mutations. A thorough understanding subtype-specific features vital accurate diagnosis, stratification, personalized management patients. Future improvements techniques standardization are needed enhance decision-making cancer.

Background: Thyroid cancer is a slow-growing tumor with excellent oncological outcomes. However, few patients unexpectedly severe outcomes are usually ignored.Current Concepts: Radioactive iodine therapy the mainstay treatment for differentiated thyroid distant metastasis. The refractoriness to radioactive has been overcome by emergence of targeted agents. First, multikinase inhibitors (sorafenib and lenvatinib) targeting growth factor pathway were developed approved as anticancer agents advanced cancer, regardless their genetic features. With progress in sequencing techniques, backgrounds tumors have unveiled new targets, including rearrangements during transformation tropomyosin receptor kinase. Special attention should be paid national health insurance coverage systemic therapeutics studies.Discussion Conclusion: New drugs introduced treat previously untreatable cancers. cost these increased developments, only first-line still covered insurance. These medical advances will remain an illusion clinics if improvements healthcare policies do not accompany them.

While fine needle aspiration cytology (FNAC) plays an essential role in the screening of thyroid nodules, core biopsy (CNB) acts as alternative method to address FNAC limitations. However, diagnosing CNB samples can be challenging due variations background and levels experience. Effective training is indispensable mitigate this challenge. We aim evaluate impact educational program on improving accuracy diagnostics.
