The detection of drug-specific activation T cells in the lymphocyte transformation test (LTT) is mainly based on cell proliferation or cytokine secretion. However, LTT presents with a varying sensitivity and specificity. aim our study was to analyse genome wide gene expression PBMC identify drug allergy-specific regulation patterns. Additionally, differences related allergy-inducing type allergy (immediate/delayed) were investigated. Trimethoprim-sulfamethoxazole (TMP-SMX) is a non-β-lactam antibiotic commonly used in pediatrics to treat infections and as prophylactic medication. Hypersensitivity TMP-SMX generally non-immediate, immediate allergic reactions, including anaphylaxis, are rare. This case report details six-year-old girl who experienced anaphylaxis TMP-SMX. Her skin prick test with was negative, but she developed drug provocation (DPT). An additional basophil activation (BAT) for the combination tablet evaluated confirmed positive. underscores need alternative diagnostic methods like BAT, which pose lower risk than DPT. The findings suggest that BAT could offer safer approach, though more studies required validate its use allergy diagnosis.

Introduction Cefuroxime allergy may present as a delayed-type reaction or an immunoglobulin (Ig)E-mediated immediate-type anaphylactic reaction. The basophil activation test (BAT) is diagnostic tool for cefuroxime-induced reactions, whereas the lymphocyte transformation (LTT) typically applied in drug allergy. This study aimed to compare results of BAT and LTT 15 patients with reactions considered confirmed. pharmacoepidemiological part analyze spontaneous reports cefuroxime-associated European adverse database (EudraVigilance). Methods In EudraVigilance, 668 Economic Area (EEA) between 2010 2023 were analyzed, 182 (27.2%) these originating from Germany. performed according standard protocols. Except one patient, all prior skin tests, thereafter. Hypersensitivity reactions (HSR) are reported for the macrolides, lincosamides, and streptogramins (MLS) antibiotic family. Data about cross-reactivity among between MLS remain scarce or controversial. The aim of this study was to provide an overview hypersensitivity cross-reactions MLSs based on data extracted from French National Pharmacovigilance Database (FPVD). Cases HSR January 1985 December 2019 were FPVD using standardized MedDRA queries (SMQ). including allergological test involving multiple giving at least one positive result included. Of 8394 cases reviewed, 149 mainly involved pristinamycin (n = 83; 53.2%) spiramycin 31; 19.9%). immediate in 54 delayed 94 cases.
