Background: For more than 20 years, China's basic medical insurance system has realized the span from "system construction" to "improving quality and efficiency". At present, as core issue of design, current financing mechanism is difficult adapt situation tasks. Objectives: Based on theory social equity, this paper uses literature analysis comparative research analyze forms health in four developed countries: Germany, United States, Singapore Kingdom. On basis, it makes a systematic reference reflection China. Methods: The author adopts method study. Through existing theoretical mechanism, conceptual connotation dimension are further summarized analyzed, which provides basis for writing ideas paper. By analyzing comparing typical practice model countries, ideas, main practices, direction goals, principle adherence, implementation results, realistic difficulties obstacles obtaining discussed. This analyzes development status between China countries by means classification, induction comparison, puts forward optimization path based experience latter's fair practice. Results: proposes that funding three problems: An unbalanced structure unreasonable sharing responsibility; low level coordination unfair method; dynamic growth endogenous not yet been established. Main Contribution Evidence-Based Practice: international experience, some enlightening reform measures solve problems China: first, through reasonable definition rights responsibilities multi-stakeholder, construct appearance implementation; Second, fixed methods establish linked income; third improve actuarial fund adjustment promote sustainable funds. We will optimize high-quality insurance.

Objective To ascertain the prevalence of asthma attacks among archivists and identify associated occupational factors in this understudied professional population. Methods We conducted a cross-sectional, questionnaire-based study 1,002 archival workers. A multiple logistic regression was to association between exposures. The Strobe Protocol applied. Results 999 workers were included final analysis with 33.3%. Main (OR [95% CI]) presence chemically irritating odors (2.152 [1.532–3.024]), mold (1.747 [1.148–2.658]), insects (1.409[1.041–1.907]). significant synergistic effect observed chemical irritants mold, odds ratio 7.098 (95% CI, 4.752–10.603). Conclusion There high workers, an under-studied Chemical irritants, molds their attacks. Notably, study’s data has revealed strong synergy = 7.098) workplace. While existing international literature on specific interaction remains somewhat limited, previous studies have already demonstrated potential for such as sulfur dioxide ozone, synergistically interact inhalable allergens, including fungi, dust mites. Consequently, seems exacerbate symptoms perpetuate untreated exposure. Adenoid Cystic Carcinoma (ACC) is a slow-growing, locally invasive malignant tumour, with deceptively benign histological appearance, high probability of local recurrence and distant metastasis. The tumour comprises basaloid cells small, angular hyperchromatic nuclei scarce cytoplasm arranged into three significant prognostic patterns: cribriform, tubular, solid. In some tumours, dedifferentiation to high-grade form occurs. authors have reviewed numerous studies which attempted demonstrate accurate features, but these often produced conflicting results. literature review suggests that Analysis microarray gene expression profiles provided new potential diagnostic markers. However, tumor grade, lymph node metastasis, invasion major nerves, resection margin status continue be the most consistent predictors prognosis. optimal treatment ACC has not yet been fully established, although advocate using surgical excision postoperative radiotherapy. Careful staging grading documentation perineural remain important tools. incurable, patients will eventually succumb recurrence, or both. clinical course disease can surprisingly heterogeneous, surviving decades others only months. combination surgery radiotherapy in period shown best locoregional control. Contribution Evidence-Based healthcare: This contributes better understanding diagnosed cystic adenocarcinoma head neck, regard curative radiotherapy, as part proposed even reduce effects disease.

Gene editing with CRISPR-Cas9 technology revolutionizes modern medicine by enabling precise DNA modifications. Initially discovered as a bacterial defense mechanism, the Cas9 enzyme, guided RNA, can target and cut specific sites, allowing for gene editing. Applications include genetic engineering, functional studies, potential treatments diseases like cancer. Notably, in 2023, was approved treating sickle cell anaemia significant results, despite challenges long-term security high costs. allows correcting defects, diseases, improving agricultural crops. It regulate transcription through CRISPRi system, using an inactive to interfere expression without permanently altering DNA. This gene-editing tool shows promise therapy, potentially curing HIV-1, disease, haemophilia B. However, off-target mutations efficient delivery of CRISPR/Cas9. Precise site selection dosage control are crucial, tools CasOT helping identify prevent unwanted mutations. requires PAM sequence function, narrowing its targets genome but increasing specificity. Production gRNA faces due mRNA processing, alternatives artificial RGR showing promise. Efficient methods still needed, current techniques involving RNA injection. Future applications improvements, ongoing research essential overcoming ensuring safety accuracy. Global collaboration is vital ethical use this technology.
