Aim . To analyze early disorders of carbohydrate metabolism and insulin resistance (IR) at different compliance levels in young people. Material methods Ninety-four patients (45 men/49 women; median (Me)=33 [28,7-38] years) were examined. This prospective observational study used data the questionnaire quantitative assessment adherence to treatment (QAA-25), fasting plas­ma glucose levels, oral tolerance test, glycated hemo­globin (HbA 1c ), Homeostasis Model Assessment Insulin Resistance (HOMA-IR). Statistical processing was carried out IBM SPSS Statistics 26. Results Only 15,8% had high medical support, 8,9% — lifestyle modification (LM) general compliance, 10,9% drug therapy. The means for most types adherences low range. In medium LM groups, over 18 months number with elevated HbA increased from 7,1 18,8% (p=0,021) IR 12% 31,8% (p=0,000). overall compared ones, probability increase raised by 5,5 times, hyperinsulinemia 1,16 times. Conclusion an 18-month study, adults showed incidence hyper­insulinemia, IR, prediabetes.

Aim . To study the relationship and predictive value of integral metabolic indices obesity in identification ectopic obesity. Material methods The included 326 patients (146 men 180 women, mean age 61±9 years). Anthropometric data, degree obesity, body mass index (BMI), waist circumference (WC), sagittal abdominal diameter (SAD), (BAI), visceral (VAI), lipid accumulation (LAP), triglycerides-glucose (TyG) was assessed. Perivascular adipose tissue (PVAT) volume, pericardial (PAT) volume thickness perirenal fat (PRF) were assessed with CT. Results Patients divided into groups: isolated (iEO) (n=17); (iAO) (n=74); EO variants: (PCO) (n=31), perivascular (PVO) (n=22) or (PRO) (n=33) AO; mixed AO (mEO+AO) (n=117), without (n=32). Significantly higher BMI, WC, SAD, BAI found individuals PCO+AO, PRO+AO mEO+AO. highest values LAP PVO+AO, PRO+AO. There a correlation between depots WC (r=0,62 for PAT, r=0,55 PVAT r=0,39 PRF, p=0,01) SAD (0,429, 0,329 0,435, respectively, p=0,01). Background — Chronic heart failure with preserved ejection fraction (CHFpEF) develops as a result of many diseases that lead to significant metabolic disorders. Given the heterogeneity this group patients, therapeutic options for syndrome are extremely limited. In regard, it seems promising study metabolomic profile in patients CHFpEF identify biomarkers, examine their roles pathogenesis syndrome, and search potential targets targeted therapy. Objective The aimed at testing correlation between threonine level features clinical course CHFpEF. Methods included total 154 patients: 82 CHFpEF, 45 hypertension and/or coronary artery disease (comparison group), 27 healthy volunteers (control group). Threonine levels were assessed using high-performance liquid chromatography-mass spectrometry. Results concentration was significantly reduced (1) vs. comparison (2) control (3): p<0.001; p1-3<0.001; p2-3=0.037). A reduction characteristic diabetes mellitus without (p=0.029).
