More than 5 years have passed since the Diffusion Tensor Image Analysis ALong Perivascular Space (DTI-ALPS) method was proposed with intention of evaluating glymphatic system. This is handy due to its noninvasiveness, provision a simple index in straightforward formula, and possibility retrospective analysis. Therefore, ALPS adopted evaluate system for many disorders studies. The purpose this review look back discuss at moment.

The diffusivity along perivascular spaces based on diffusion tensor imaging (DTI-ALPS index) was measured assess pathway. presence CSVD were estimated using score (points from 0 4) modified 4), which driven by 4 features CSVD, including white matter hyperintensity (WMH), enlarged (EPVS), lacunes, microbleeds. Brain atrophy (BA) also evaluated. Binary or ordinal logistic regression analyses carried out relationships DTI-ALPS index CSVD.The mean age 61.3 (SD 6.6) years, 1019 (45.9%) participants men. average 1.67±0.14. Individuals first quartile (Q1) had higher risks (OR 1.77, 95% CI 1.33 2.35, p<0.001), (odds ratio (OR) 1.80, 1.38 2.34, total burden (common OR (cOR) 1.89, 1.43 2.49, p<0.001) (cOR 1.95, 1.51 2.50, compared those fourth (Q4). Objectives: Dementia is a clinical syndrome caused by multiple etiologies, usually manifests with progressive and diffuse brain dysfunction. The activity of the human glymphatic system was evaluated in cases dementia diffusion tensor image analysis along perivascular space (DTI-ALPS). Methods: We recruited 28 healthy subjects 77 patients, including 38 Alzheimer’s disease (AD),18 mild cognitive impairment (MCI), normal controls (NC) 21 vascular (VCI). All participants underwent DTI scanning. Diffusivities X, Y Z axes were obtained lateral ventricle body plane all subjects. assessed diffusivity spaces, as well projection fibers association fibers, respectively, order to acquire an DTI-ALPS-index correlated them mini mental state examination (MMSE) montreal assessment (MOCA) scores using partial correlation which influence age controlled. Results: AD, MCI, VCI patients showed significantly lower (p < 0.001) compared NC. Besides, group had higher than AD = 0.007). There significant positive between MMSE MOCA (the effect controlled), showing that water spaces associated dementia. The Dzassoc led reduced VCI, while Dxassoc contributed decrease AD. Conclusion: evaluation DTI-ALPS demonstrates decreased DTI-ALPS-index. Different from show drainage disorder rather impairment. Perivascular spaces have been involved in neuroinflammatory and neurodegenerative diseases. Upon a certain size, these can become visible on magnetic resonance imaging (MRI), referred to as enlarged perivascular (EPVS) or MRI-visible (MVPVS). However, the lack of systematic evidence etiology temporal dynamics MVPVS hampers their diagnostic utility MRI biomarker. Thus, goal this review was summarize potential etiologies evolution MVPVS.In comprehensive literature search, out 1,488 unique publications, 140 records assessing etiopathogenesis were eligible for qualitative summary. 6 included meta-analysis assess association between brain atrophy.Four overarching partly overlapping proposed: (1) Impairment interstitial fluid circulation, (2) Spiral elongation arteries, (3) Brain atrophy and/or myelin loss, (4) Immune cell accumulation space. The patients with diseases did not support an volume measures [R: -0.15 (95%-CI -0.40-0.11)]. Based few mostly small studies tumefactive vascular diseases, is slow.Collectively, study provides high-grade dynamics. Although several emergence proposed, they are only partially supported by data. BACKGROUND: Studies of the neurovascular contribution to dementia have largely focused on cerebral small vessel disease (CSVD), but role intracranial atherosclerotic (ICAD) remains unknown in general population. The objective this study was determine risk incident from ICAD after adjusting for CSVD and cardiovascular factors a US community-based cohort. METHODS: We acquired brain magnetic resonance imaging examinations 2011 through 2013 1980 Black White participants ARIC (Atherosclerosis Risk Communities), prospective cohort conducted 4 communities. Magnetic included high-resolution wall angiography identify ICAD. Of these participants, 1590 without dementia, missing covariates, with adequate image quality were followed 2019 dementia. Associations between assessed using Cox proportional hazard ratios adjusted (characterized by white matter hyperintensities, lacunar infarctions, microhemorrhages), APOE4 genotype (apolipoprotein E gene ε4), factors. RESULTS: mean age (SD) 77.4 (5.2) years. detected 34.6% participants. Perivascular spaces (PVS) and blood-brain barrier (BBB) disruption are two key features of cerebral small vessel disease (cSVD) neurodegenerative diseases that have been linked to cognitive impairment involved in the waste clearance system. Magnetic resonance imaging (MRI) offers possibility study these pathophysiological processes noninvasively vivo. This educational review provides an overview MRI techniques used assess PVS functionality BBB disruption. MRI-visible can be scored on structural images by either (subjectively) counting or (automatically) delineating PVS. We highlight emerging (diffusion) measure proxies perivascular fluid its movement, which may provide a more comprehensive understanding role diseases. For measurement disruption, we explain most commonly technique, dynamic contrast-enhanced (DCE) MRI, as well recently developed technique based arterial spin labeling (ASL). DCE ASL thought complementary characteristics BBB. Furthermore, describe clinical studies utilized cSVD diseases, particularly Alzheimer's (AD). These demonstrate dysfunction insight into large overlap, but also differences between AD. Overall, valuable insights mechanisms underlying potential markers for progression treatment response.
