Abstract This study aimed to report the most current data on incidence and disability-adjusted life years (DALY) associated with osteoarthritis in China from 1990 2019. Publicly available modelled Global Burden of Disease Study (GBD) 2019 were used. The DALY, due China, stratified by sex, trends risk factors, assess age, period, cohort effects long-term DALY We found that age-standardized rates are higher than average levels Asia, Africa, Oceania. In 2019, number cases was 10,681,311, an increase 132.66% compared 1990. 4,724,885 person-years, which 159.70% 750.96/100,000,332.19/100,000. High body-mass as factors for population attributable proportion (PAF) increasing steadily three types high low knee, hand, hip. Age–period–cohort model showed rate shows a trend first then decreasing age; concurrently, increased age. For period effect, we ratio ( RR ) kept born before 2005–2009, then, it gradually reduced year birth after 2005–2009. As almost has no change, while burden impact substantial increasing. Adopting suitable control preventive community measures reduce modifiable is needed future China. Key Points • paper analyzes disease time discusses influence perspectives .

Knee osteoarthritis (OA) has long been considered as a cartilage disease. However, it is now well established that defects are the ultimate OA feature only. Several studies investigating mechanisms of onset/progression have recognized infrapatellar fat pad an active OA-player but also possible source stem cells with regenerative potential for cartilage. Is really like that? cautious approach should be adopted.

Degenerative orthopaedic diseases pose a notable worldwide public health issue attributable to the global aging population. Conventional medical approaches, encompassing physical therapy, pharmaceutical interventions, and surgical methods, face obstacles in halting or reversing degenerative process. In recent times, exosome-based therapy has gained widespread acceptance popularity as an effective treatment for diseases. This therapeutic approach holds potential "cell-free" tissue regeneration. Exosomes, membranous vesicles resulting from fusion of intracellular multivesicles with cell membrane, are released into extracellular matrix. Addressing challenges such rapid elimination natural exosomes vivo limitation drug concentration can be effectively achieved through various strategies, including engineering modification, gene overexpression biomaterial binding. review provides concise overview source, classification, preparation methods exosomes, followed by in-depth analysis their functions applications. Skeletal diseases impose a considerable burden on society. The clinical and tissue-engineering therapies applied to alleviate such frequently result in complications are inadequately effective. Research has shifted from conventional based mesenchymal stem cells (MSCs) exosomes derived MSCs. Exosomes natural nanocarriers of endogenous DNA, RNA, proteins, lipids have low immune clearance rate good barrier penetration allow targeted delivery therapeutics. MSC-derived (MSC-exosomes) the characteristics both MSCs exosomes, so they can immunosuppressive tissue-regenerative effects. Despite advances our knowledge MSC-exosomes, their regulatory mechanisms functionalities unclear. Here we review therapeutic potential MSC-exosomes for skeletal diseases.

Traditional Chinese Medicine (TCM) believes that OA belongs to category of “Bi syndrome” and “bone Bi syndrome”. The etiology pathogenesis lie deficiency liver kidney, Qi blood, external exposure wind, cold, dampness. Epimedium yang-reinforcing herb TCM, which can tonify strengthen muscles bones, dispel cold dampness, treat both symptoms root cause In addition, contains large number ingredients. Through modern science technology, more than 270 compounds have been found , among flavonoids are main active Therefore, our study will review effects mechanisms genus treating from two aspects: (1) Introduction its ingredients; (2) Effects ingredients relevant signaling pathways, order provide ideas for treatment.
