Diabetic kidney disease (DKD) is a highly prevalent condition worldwide. It represents one of the most common complications arising from diabetes mellitus (DM) and leading cause end-stage (ESKD). Its development involves three fundamental components: hemodynamic, metabolic, inflammatory axes. Clinically, persistent albuminuria in association with progressive decline glomerular filtration rate (GFR) defines this disease. However, as these alterations are not specific to DKD, there need discuss novel biomarkers its pathogenesis which may aid diagnosis, follow-up, therapeutic response, prognosis

The aim of this study is to analyze the effect serum metabolites on diabetic nephropathy (DN) and predict prevalence DN through a machine learning approach. dataset consists 548 patients from April 2018 2019 in Second Affiliated Hospital Dalian Medical University (SAHDMU). We select optimal 38 features least absolute shrinkage selection operator (LASSO) regression model 10-fold cross-validation. compare four algorithms, including extreme gradient boosting (XGB), random forest, decision tree, logistic regression, by AUC-ROC curves, calibration curves. quantify feature importance interaction effects predictive Shapley additive explanation (SHAP) method. Abstract Background In the last years, classical pattern of diabetic kidney disease (DKD) has been partially overcome, because uncovering a new DKD phenotype with significant renal dysfunction without presence albuminuria: non-albuminuric (NA-DKD). To date, cardiovascular risk associated this is still debated. We investigated and injury profile NA-DKD subjects in comparison other phenotypes. Methods Pulse wave velocity (PWV), intima-media thickness, carotid atherosclerotic plaque, resistive index (RRI), panel urinary biomarkers were evaluated 160 type 2 diabetes, stratified according to estimated glomerular filtration rate (eGFR) albumin creatinine ratio (UACR) into four groups: controls (UACR < 30 mg/g eGFR ≥ 60 mL/min/1.73 m ), A-DKD (Albuminuric-DKD, UACR AL-DKD (Albuminuric Low eGFR-DKD; ). Results Subjects showed higher PWV (11.83 ± 3.74 m/s vs. 10.24 2.67 m/s, P = 0.045), RRI (0.76 0.11 0.71 0.09, 0.04), prevalence plaque (59% 31%, 0.009) compared controls. These characteristics similar those AL-DKD, whereas was closer After multiple regression analyses, we found that RRI, turn influenced by (β − 0.01, 0.01), one major determinants 9.4, 0.02). Diabetic nephropathy (DN) is one of the most common complications diabetes mellitus. After development DN, patients will progress to end-stage renal disease, which associated with high morbidity and mortality. Here, we developed early-stage diagnostic biomarkers detect DN as a strategy for intervention. For model, Zucker diabetic fatty rats were used phenotyping. The results revealed that showed significantly increased blood glucose, urea nitrogen (BUN), serum creatinine levels, accompanied by severe kidney injury, fibrosis microstructural changes. In addition, urinary excretion injury molecule-1 (KIM-1) neutrophil gelatinase-associated lipocalin (NGAL). Transcriptome analysis new biomarkers, such complementary component 4b (C4b), factor D (CFD), C-X-C motif chemokine receptor 6 (CXCR6), leukemia inhibitory (LIF) identified. Furthermore, they found in urine DN. Since these detected patients, selected markers could be early diagnosis chronic nephropathy.

This study assessed the efficacy of INV-202, a novel peripherally restricted cannabinoid type-1 receptor (CB1R) inverse agonist, in streptozotocin-induced diabetes nephropathy mouse model.Diabetes was induced 8-week-old C57BL6/J male mice via intraperitoneal injection streptozotocin (45 mg/kg/day for 5 days); nondiabetic controls received citrate buffer. Diabetic were randomized to 3 groups based on blood glucose, polyuria, and albuminuria, administered daily oral doses 28-days INV-202 at 0.3 or mg/kg vehicle.INV-202 did not affect body weight but decreased kidney compared with vehicle group. While polyuria unaffected by treatment, urinary urea (control 30.77 ± 14.93; 189.81 31.49; (0.3 mg/kg) 127.76 20; (3 93.70 24.97 mg/24h) albumin 3.06 0.38; 850.08 170.50; 290.65 88.70; 111.29 33.47 µg/24h) excretion both vehicle-treated diabetic mice.
