Hepatocellular
carcinoma
is
the
most
common
primary
malignancy
of
liver,
with
hepatocellular
differentiation.
It
ranked
sixth
among
cancers
worldwide
and
third
leading
cause
cancer-related
deaths.
The
important
etiological
factors
discussed
here
are
viral
infection
(HBV,
HCV),
exposure
to
aflatoxin
B1,
metabolic
syndrome,
obesity
(as
an
independent
factor).
Directly
or
indirectly,
they
induce
chromosomal
aberrations,
mutations,
epigenetic
changes
in
specific
genes
involved
intracellular
signaling
pathways,
responsible
for
synthesis
growth
factors,
cell
proliferation,
differentiation,
survival,
metastasis
process
(including
epithelial–mesenchymal
transition
expression
adhesion
molecules),
angiogenesis.
All
these
disrupted
molecular
mechanisms
contribute
hepatocarcinogenesis.
Furthermore,
equally
interaction
between
tumor
cells
components
microenvironment:
inflammatory
macrophages—predominantly
a
pro-tumoral
role—hepatic
stellate
cells,
tumor-associated
fibroblasts,
cancer
stem
extracellular
vesicles,
matrix.
In
this
paper,
we
reviewed
biology
intricate
hepatocarcinogenesis,
highlighted
how
certain
pathways
can
be
pharmacologically
influenced
at
various
levels
molecules.
Additionally,

several
examples

clinical

described

Abstract
Immune
checkpoints
(ICPs)
can
promote
tumor
growth
and
prevent
immunity‐induced
cancer
cell
apoptosis.
Fortunately,
targeting
ICPs,
such
as
programmed
death
1
(PD‐1)
or
cytotoxic
T
lymphocyte
associated
protein
4
(CTLA‐4),
has
achieved
great
success
in
the
past
few
years
gradually
become
an
effective
treatment
for
cancers,
including
hepatocellular
carcinoma
(HCC).
However,
many
patients
do
not
respond
to
ICP
therapy
due
acquired
resistance
recurrence.
Therefore,
clarifying
specific
mechanisms
of
development
HCC
is
very
important
enhancing
efficacy
anti‐PD‐1
anti‐CTLA‐4
therapy.
In
particular,
