Amyotrophic lateral sclerosis (ALS) continues to pose a significant challenge due the disease complexity and heterogeneous manifestations. Despite recent drug approvals, there remains critical need for development of more effective therapies. This review explores underlying mechanisms involved; including neuroinflammation, glutamate mediated excitotoxicity, mitochondrial dysfunction, hypermetabolism, how researchers are trying develop novel drugs target these pathways. While progress has been made, unmet ALS patients highlights urgency continued research resource allocation in pursuit treatments.
