Abstract Aims The concept of “atrial cardiomyopathy” (AtCM) had been percolating through the literature since its first mention in 1972. Since then, publications using term were sporadic until decision was made to convene an expert working group with representation from four multinational arrhythmia organizations prepare a consensus document on atrial cardiomyopathy 2016 (EHRA/HRS/APHRS/SOLAECE cardiomyopathies: definition, characterization, and clinical implication). Subsequently, AtCM have increased progressively. Methods results present elaborates further implement simple staging system (AtCM stages 1–3) by integrating biomarkers, geometry, electrophysiological changes. However, proposed needs validation. Importantly, it is clearly stated that presence might serve as substrate for development fibrillation (AF) AF may accelerates substantially, but per se be viewed separate entity. Conclusion Thus, serves statement European Heart Rhythm Association (EHRA) ESC, Society (HRS), Asian Pacific (APHRS), Latin American (LAHRS) contribute evolution concept.

Atrial fibrillation (AF) is the most common supraventricular arrhythmia affecting up to 1% of general population. Its prevalence dramatically increases with age and could reach ∼10% in elderly. The management AF a complex issue that object extensive ongoing basic clinical research, it depends on its genetic epigenetic causes, varies considerably geographically also according ethnicity. Mechanistically, over last decade, Genome Wide Association Studies have uncovered 100 loci associated AF, shown European ancestry elevated risk AF. These AF-associated revolve around different types disturbances, including inflammation, electrical abnormalities, structural remodeling. Moreover, discovery regulatory mechanisms, involving non-coding RNAs, DNA methylation histone modification, has allowed unravelling what modifications reshape processes leading arrhythmias. Our review provides current state field regarding identification functional characterization AF-related networks, ethnic differences. We discuss clear emerging connections between regulation pathophysiological mechanisms

However, several promising candidates there are exciting developments repurposing reformulating already existing indications related arrhythmias. This review discusses key conceptual considerations new agents, summarizes compounds formulations currently clinical rhythm control atrial fibrillation, highlights drug repurposing. Finally, future directions AAD discussed. Chronic kidney disease (CKD) is associated with a higher risk of atrial fibrillation (AF). The mechanistic link between CKD and AF remains elusive. Interleukin (IL)-1β, main effector 'NLR-family pyrin domain-containing 3' (NLRP3) inflammasome activation, key modulator conditions inflammation, such as CKD. Circulating IL-1β levels were elevated in CKD-patients vs sinus rhythm. Moreover, NLRP3-activity was enhanced atria CKD-patients. To elucidate the role NLRP3/IL-1β signaling pathogenesis CKD-induced AF, wildtype (WT) Nlrp3-/- mice subjected to two-stage subtotal nephrectomy protocol induce 4-weeks post-surgery, serum tissue increased WT-CKD (vs WT-sham) mice. susceptibility pacing-induced longer AF-duration electrical remodeling, enlarged atria, fibrosis. Genetic inhibition NLRP3 or neutralizing anti-IL-1β antibodies effectively reduced IL-1β-levels, normalized left dimensions, fibrosis AF-incidence. These data suggest that creates substrate for development by activating which structural remodeling. Neutralizing may be beneficial prevention AF.

Abstract: Postoperative atrial fibrillation (POAF) after cardiac surgery is associated with elevated morbidity and mortality. Although current prediction models have limited efficacy, several perioperative interventions can reduce patients' risk of POAF.
