Abstract Reperfusion therapy, employed in the treatment of acute stroke, frequently proves to be inadequate addressing primary brain tissue injury and may even give rise secondary damage. The study introduces a satellite nanoparticle platform named MEps, which combine neural repair properties bone marrow mesenchymal stem cell exosomes (Exos) with inflammatory site‐targeting abilities macrophage membranes (MMs). MMs Exos MEps act like satellites, ensuring precise positioning information transmission. rapidly form protective barrier on damaged cerebral vascular endothelial cells through interaction adhesion molecules their receptors, blocking infiltration neutrophils. Abstract Immunoinflammation is associated with the development of post-stroke cognitive impairment (PSCI), however, peripheral immunity has not been fully explored. We aimed to investigate association between PSCI and immune indicators, including neutrophil, lymphocyte, mononuclear percentages counts; systemic inflammation index; platelet-to-lymphocyte ratio; neutrophil-to-lymphocyte ratio (NLR); lymphocyte-to-monocyte ratio. A total 224 patients acute minor ischemic stroke or transient attack 6–12 months follow-up were included. was defined as a Montreal Cognitive Assessment score < 22 during period. performed logistic regression, subgroup analyses based on age sex, further established predictive models. found that increased innate indicators (neutrophils, neutrophil percentage) risk PSCI, whereas adaptive indicator (lymphocytes) protective against especially in aged 50–65 years.
