Depression is the most prevalent mental disorder in world associated with huge socio-economic consequences. While depressive-related symptoms are well known, molecular mechanisms underlying disease pathophysiology and progression remain largely unknown. The gut microbiota (GM) emerging as a key regulator of central nervous system homeostasis by exerting fundamental immune metabolic functions. In turn, brain influences intestinal microbial composition through neuroendocrine signals, within so-called microbiota-brain axis. balance this bidirectional crosstalk important to ensure neurogenesis, preserve integrity blood-brain barrier avoid neuroinflammation. Conversely, dysbiosis permeability negatively affect development, behavior, cognition. Furthermore, although not fully defined yet, changes GM depressed patients reported influence pharmacokinetics common antidepressants affecting their absorption, metabolism, activity. Similarly, neuropsychiatric drugs may shape turn an impact on efficacy toxicity pharmacological intervention itself. Consequently, strategies aimed at re-establishing correct homeostatic (i.e., prebiotics, probiotics, fecal transplantation, dietary interventions) represent innovative approach improve pharmacotherapy depression. Among these, probiotics Mediterranean diet, alone or combination standard care, hold promise for clinical application. Therefore, disclosure intricate network between depression will give precious insights diagnostic therapeutic approaches towards depression, profound implications drug development practice.

The emergence and rapid spread of antibiotic resistance seriously compromise the clinical efficacy current therapies, representing a serious public health threat worldwide. Generally, drug-susceptible bacteria can acquire through genetic mutation or gene transfer, among which horizontal transfer (HGT) plays dominant role. It is widely acknowledged that sub-inhibitory concentrations antibiotics are key drivers in promoting transmission resistance. However, accumulating evidence recent years has shown addition to antibiotics, non-antibiotics also accelerate genes (ARGs). Nevertheless, roles potential mechanisms non-antibiotic factors ARGs remain largely underestimated. In this review, we depict four pathways HGT their differences, including conjugation, transformation, transduction vesiduction. We summarize accounting for enhanced underlying molecular mechanisms. Finally, discuss limitations implications studies.

Abstract Background There is a growing body of evidence suggesting that disturbance the gut-brain axis may be one potential causes major depressive disorder (MDD). However, effects antidepressants on gut microbiota, and role microbiota in influencing antidepressant efficacy are still not fully understood. Results To address this knowledge gap, multi-omics study was undertaken involving 110 MDD patients treated with escitalopram (ESC) for period 12 weeks. This conducted within cohort compared to reference group 166 healthy individuals. It found ESC ameliorated abnormal blood metabolism by upregulating MDD-depleted amino acids downregulating MDD-enriched fatty acids. On other hand, use showed relatively weak inhibitory effect leading reduction microbial richness functions. Machine learning-based integrative analysis revealed contributed changes plasma metabolites associated several such as tryptophan its microbiota-derived metabolite, indole-3-propionic acid (I3PA). Notably, significant correlation observed between baseline clinical remission at week 12. Compared non-remitters, individuals who achieved had higher richness, lower dysbiosis score, more complex well-organized community structure bacterial networks their microbiota. These findings indicate resilient remitters. Furthermore, we also demonstrated it composition itself, but rather presence sporulation genes could predict likelihood following treatment. The predictive model based these an area under curve (AUC) performance metric 0.71. Conclusion provides valuable insights into mechanism treatment MDD. represent advancement understanding intricate relationship among antidepressants, metabolome. Additionally, offers microbiota-centered perspective can potentially improve practice.
