Abstract: This narrative state-of-the-art review paper describes the progress in understanding and treatment of Posttraumatic Stress Disorder (PTSD). Over last four decades, scientific landscape has matured, with many interdisciplinary contributions to its diagnosis, etiology, epidemiology. Advances genetics, neurobiology, stress pathophysiology, brain imaging have made it apparent that chronic PTSD is a systemic disorder high allostatic load. The current state includes wide variety pharmacological psychotherapeutic approaches, which are evidence-based. However, myriad challenges inherent disorder, such as individual barriers good outcome, comorbidity, emotional dysregulation, suicidality, dissociation, substance use, trauma-related guilt shame, often render response suboptimal. These discussed drivers for emerging novel including early interventions Golden Hours, interventions, medication augmentation use psychedelics, well targeting nervous system. All this aims improve symptom relief clinical outcomes. Finally, phase orientation recognized tool strategize position step progression pathophysiology. Background The psychological treatment of comorbid post-traumatic stress disorder (PTSD) and substance use (SUD) is clinically challenging, outcomes are often poor. Objective This paper describes a systematic review meta-analysis which sought to establish the current efficacy for number established approaches adults adolescents, in comparison interventions SUD alone, or other active approaches, following pre-registered protocol.Method followed PRISMA Cochrane Collaboration guidelines. Data extraction risk bias judgements using criteria were undertaken by all authors. Primary PTSD severity post-treatment. quality findings was assessed GRADE. Following comprehensive search, conducted 13 September 2021, 27 studies included. Results We found relatively high level dropout across studies. In our main comparisons, we no benefits present-focused aimed at improving coping skills beyond those SUD-only interventions. modest trauma-focused intervention plus post-treatment (standardized mean difference (SMD) = −0.36, 95% confidence interval (CI) −0.64, −0.08), 6–13 months (SMD −0.48, CI −0.81, −0.15) alcohol −0.23, −0.44, −0.02). Abstract Background Trauma-focused therapies (TFTs) are first-line treatments for posttraumatic stress disorder (PTSD). However, TFTs under-utilised, partly due to clinicians’ and patients' fear that TFT is too challenging or harmful. We review the qualitative studies on how adults with PTSD experience enhance understanding of user perspectives, therapeutic processes, outcomes. Methods PubMed, PsychINFO PTSDPubs were searched between October 1st November 30th, 2021. Study quality assessments undertaken, analysed using a descriptive-interpretative approach. Nine included. Results The analysis resulted in identification four key domains, representing temporal sequence stages: Overcoming ambivalence towards TFT, Experience treatment elements, Motivation dropout/retention, Perceived changes post-treatment. Conclusion Although many participants reported high levels distress considered dropping out, only minority did eventually drop out most patients expressed hardships therapy necessary improvement. Establishing safe environment working was essential retention. The purpose the study was assess feasibility, safety, acceptability, preliminary outcomes at end 3-month follow-up. Methods: A total N = 56 eligible veterans CPTSD were either ESTAIR (n 28) or TAU 28). Linear mixed models conducted severity, primary outcome, measured by International Trauma Questionnaire (ITQ). Results: Treatment dropout low equivalent (18% vs. 11%; χ2 (1) 1.19, p 0.275), retention high, supporting feasibility study. No serious adverse effects very few occurred, none which deemed related provided significantly greater reduction severity across time for ITQ (p < 0.001) DSO symptoms. pre-to-post effect sizes large (PTSD d 1.78; 2.00).
